Kidney Involvement in Systemic Diseases

Many diseases that affect the whole body also injure the kidneys. The commonest are diabetes and high blood pressure, followed by autoimmune conditions such as lupus, ANCA vasculitis, scleroderma, Sjogren syndrome and rheumatoid arthritis, and blood disorders such as myeloma.

Kidney involvement is often silent. It may be discovered only as protein or blood in the urine or a rising creatinine, yet it strongly influences long-term outlook. That is why people with these conditions need regular urine and blood checks.

Symptome

• Frequently none in the early stages
• Foamy urine or visible blood in the urine
• Swollen ankles or puffy eyes
• New or harder-to-control high blood pressure
• Tiredness, poor appetite and itching as function falls
• Features of the underlying disease such as joint pain, rash, fever, sinus or lung symptoms

In diabetes, high glucose and pressure within the filters cause thickening and scarring over 10 to 20 years. In lupus, immune complexes lodge in the glomeruli and cause lupus nephritis in up to half of patients. ANCA vasculitis and anti-GBM disease inflame small vessels and can destroy kidney function within weeks. Scleroderma can cause a sudden renal crisis with severe hypertension. Myeloma light chains block tubules; sarcoidosis and Sjogren syndrome inflame the tissue between tubules. Medicines used for these diseases, including NSAIDs, can add further injury.

• Urine albumin or protein to creatinine ratio: The earliest marker; checked at least yearly in diabetes and at each visit in lupus.
• Creatinine and estimated GFR: Track filtering function over time.
• Autoimmune and blood tests: ANA, anti-dsDNA, complement, ANCA, anti-GBM, serum free light chains and protein electrophoresis.
• Kidney ultrasound: Assesses size and excludes obstruction.
• Kidney biopsy: Classifies lupus nephritis, confirms vasculitis and separates active inflammation from scarring.

Behandlungsmöglichkeiten

• Control of the underlying disease: Tight but safe glucose and blood pressure control; disease-specific immunosuppression in autoimmune conditions.
• Kidney-protective medicines: ACE inhibitors or ARBs, SGLT2 inhibitors, finerenone and GLP-1 agonists in diabetic kidney disease.
• Induction and maintenance immunosuppression: Steroids with mycophenolate or cyclophosphamide for lupus nephritis, with belimumab or voclosporin in some; rituximab or cyclophosphamide for ANCA vasculitis.
• Plasma exchange: For anti-GBM disease and selected severe vasculitis.
• ACE inhibitors for scleroderma renal crisis: Started urgently in hospital.
• Dialysis and transplantation: If kidney failure develops despite treatment.
