A ventilation tube, also called a grommet or tympanostomy tube, is a tiny plastic or metal tube placed through the eardrum. It lets air into the middle ear and allows trapped fluid to clear. It is the most common operation in young children.
Tubes restore hearing quickly in glue ear and reduce repeated ear infections while they are in place. They are temporary: most fall out within 6 to 18 months, and about one child in four needs a second set.
What Ear Ventilation Tubes (Grommets) involves
Using an operating microscope through the ear canal, the surgeon makes a 2 to 3 mm slit in the lower front part of the eardrum. Thick fluid is suctioned from the middle ear, and the tube, shaped like a tiny cotton reel, is slotted into the slit so that one flange sits on each side of the drum. Nothing is stitched. Antibiotic drops may be placed. Both ears are usually done. In children with large adenoids or a second set of tubes, adenoidectomy is often added because it lowers the chance of glue ear returning.
Who is a good candidate for Ear Ventilation Tubes (Grommets)?
Most glue ear clears unaided, so tubes are offered only when fluid and hearing loss persist.
- Children with glue ear in both ears for over 3 months and documented hearing loss, speech delay or learning difficulty
- Children with frequent acute ear infections, roughly 3 in 6 months or 4 in a year
- Children with Down syndrome or cleft palate and persistent fluid, as part of specialist care
- Adults with persistent fluid or pressure damage from Eustachian tube problems, including some who struggle with flying or diving
- A retracted eardrum at risk of cholesteatoma
Es ist normalerweise nicht die richtige Wahl, wenn:
- Fluid present for under 3 months without complications
- A single mild infection history
- Adults with one-sided fluid until the back of the nose has been examined to exclude a growth
- Families who would struggle with return visits for checks
Technikoptionen
- Short-term grommets: Stay 6 to 18 months and fall out unaided. Used first in most children.
- Long-term T-tubes: Stay for years. Used after repeated sets or in adults, with a higher chance of a lasting perforation.
- Local anaesthetic insertion: Adults and cooperative older children can have tubes placed in clinic with numbing cream or drops.
- With adenoidectomy: Added for repeat tubes or blocked nose; reduces further surgery.
Was passiert während Ihrer Behandlung
In children it takes 10 to 15 minutes under a light general anaesthetic, given by mask. They go home within a few hours. Adults under local anaesthetic hear loud suction noise and may feel a brief sharp sensation. Hearing often improves at once.
Vorbereitung auf Ihre Reise
Plan for 3 to 5 days in Istanbul. Your surgeon reviews photographs and your medical history before you book, and you meet in person the day before surgery for examination, marking and consent with a professional interpreter.
Most children are back to normal the next day. A little clear or blood-stained discharge for a day or two is common. Swimming on the surface is allowed after about 2 weeks; avoid diving and soapy bath water entering the ears. Ear discharge later on means an infection around the tube and is treated with antibiotic drops. The ears are checked at about 6 weeks with a hearing test, then every 6 months until the tubes have come out and the drums have healed. Flying is safe with tubes, and is actually more comfortable.
- Back to everyday activity: 1 day
- When results show: Hearing improves immediately
- How long they last: 6 to 18 months per set
Sicherheit, Risiken und Revisionspolitik
Complications are usually minor and relate to the eardrum.
- Ear discharge from infection, which affects a fair proportion of children at some point
- A blocked tube that stops working
- Early extrusion, or a tube that stays too long and needs removing
- A small persistent perforation after the tube falls out, in a few percent, more with T-tubes
- Chalky scarring of the eardrum, which rarely affects hearing
- Return of glue ear after the tube comes out

Häufig gestellte Fragen
Does my child need earplugs?
Not for normal bathing and surface swimming in most cases. Plugs help children who get discharge after water exposure.
Is it reasonable to travel for grommets?
The operation is short, but follow-up runs for a year or more. It only makes sense if your home doctor will do the checks.
Are hearing aids an alternative?
Yes. Temporary hearing aids are a recognised option for glue ear when surgery is not wanted or not suitable.
