Ankle arthroscopy is keyhole surgery on the ankle joint. A camera about the width of a pencil is passed through small cuts at the front or back of the ankle so the surgeon can see and treat problems inside the joint.
It can remove bone spurs, loose fragments and inflamed lining, and treat cartilage damage on the talus. It cannot reverse advanced ankle arthritis, where fusion or ankle replacement is the more realistic option.
What Ankle Arthroscopy involves
You lie on your back and the ankle is gently pulled apart with a strap or traction device to open the narrow joint space. Two cuts of about 5 mm are made either side of the front tendons, taking care to avoid the superficial nerve that crosses there. Fluid is pumped in, the camera goes through one portal and fine shavers, burrs or graspers through the other. The surgeon inspects the cartilage, trims impinging bone or scar tissue, removes loose bodies and, if there is a cartilage defect, may drill small holes in the bone beneath it to stimulate a repair layer. The portals are closed with one stitch or paper strips and a padded bandage is applied.
Who is a good candidate for Ankle Arthroscopy?
It suits people whose ankle pain, catching or swelling has a mechanical cause shown on examination and MRI, and has not settled with physiotherapy.

After a simple debridement you can put weight on the foot within a few days and wear normal shoes by 2 weeks. Stitches come out at 10 to 14 days. After microfracture you stay on crutches with limited weight for 4 to 6 weeks. Swelling commonly lasts 2 to 3 months. Running usually returns at 6 to 12 weeks, later after cartilage work.
