Knochenzyste

A bone cyst is a benign, fluid-filled cavity inside a bone. The two main types occur in children and teenagers. A simple (unicameral) bone cyst is a single chamber of clear fluid, usually in the upper arm or upper thigh bone near the growth plate. An aneurysmal bone cyst is blood-filled, expands the bone and behaves more aggressively. In adults, small cysts next to arthritic joints are a different and usually minor finding.

Bone cysts are not cancer and do not spread. Their importance is that they weaken the bone, so many are discovered only when it breaks after a minor knock. Simple cysts tend to become inactive once growth finishes. Both types can return after treatment, which is why follow-up X-rays continue for a few years.

Ursachen und Risikofaktoren

The cause of simple bone cysts is not settled; the leading explanation is blocked venous drainage within growing bone, which lets fluid collect under pressure. Most aneurysmal bone cysts carry a specific gene rearrangement (USP6), so they are now regarded as true benign tumours. About a third of aneurysmal cysts form on top of another lesion such as a giant cell tumour or chondroblastoma. Neither type is caused by injury, diet or anything the child or parents did.

Behandlungsmöglichkeiten

Wenn es dringend ist

Sudden severe pain with inability to use the arm or stand on the leg suggests a fracture and needs an emergency department locally. Seek urgent review for rapidly increasing swelling, night pain, fever, unexplained weight loss, or any new weakness, numbness or bladder problems with a spinal lesion.

Treatment is planned and not urgent once any fracture has healed, so a second opinion on the scans and a scheduled operation are realistic on a visit of about 1 to 2 weeks. Because most patients are children and recurrence is common, particularly under the age of 10, X-ray follow-up every few months for at least 2 years must be arranged at home. Ask that biopsy and definitive surgery be done by the same bone tumour team.

Is a bone cyst cancer?

No. It is benign. Biopsy is sometimes advised simply to be sure an atypical lesion is not something else.

Will it go away by itself?

Simple cysts often fill in after skeletal maturity. Aneurysmal cysts rarely resolve without treatment.

Can my child play sport?

That depends on the size and site. Contact sport is usually limited while a large cyst sits in a weight-bearing bone.

How often do cysts come back?

Recurrence after treatment is common, broadly one in five to one in three depending on type, age and method, and is usually manageable with repeat treatment.
