Bone pain is a deep, boring or aching pain that seems to come from within the bone itself. Unlike muscle or joint pain it is often present at rest and at night, is not clearly tied to a particular movement, and the bone may be tender when pressed. People often find it hard to tell bone, joint and muscle pain apart, which is one task of the examination.
Most causes are benign and treatable: injury, stress fractures, vitamin D deficiency and, in children, harmless growing pains. Because infection, blood disorders and cancer can also present this way, bone pain that persists for more than a few weeks without an explanation deserves tests and should not just be masked with painkillers.

Trauma and overuse are the commonest causes, including bruising, fractures and stress fractures in runners and military recruits. Osteomalacia from vitamin D deficiency gives diffuse pain and weakness. Osteoporosis hurts only once a bone, usually a vertebra, has fractured. Other causes are osteomyelitis, Paget disease, avascular necrosis, sickle cell crises, overactive parathyroid glands, and cancers: secondary deposits from breast, prostate, lung, kidney and thyroid, multiple myeloma, leukaemia and primary tumours such as osteosarcoma and Ewing sarcoma in young people. Some medicines, including aromatase inhibitors, the first bisphosphonate infusion and G-CSF injections, cause temporary bone aches.

- History and examination: Site, night pain, relation to activity, previous cancer, medicines and general symptoms direct the tests.

- Treat the cause: The priority; painkillers are a bridge, not the plan.
- Pain relief: Paracetamol and short courses of anti-inflammatories if safe for you, with stronger medication for fracture or cancer pain.
- Rest and graded loading: Relative rest, a boot or crutches for stress fractures, then a gradual return over 6 to 12 weeks.
- Vitamin D and calcium: Replacement relieves osteomalacia pain over weeks to months.
- Bone-protecting drugs: Bisphosphonates or denosumab for osteoporosis, Paget disease and bone secondaries.
- Radiotherapy: A short course often eases pain from a cancer deposit in bone.
- Surgery: Fixation of fractures or of bones at risk of breaking, and removal of tumours or infected bone.

Seek emergency care locally for sudden severe pain after a fall or with deformity, inability to bear weight, bone pain with high fever, or back pain with leg weakness, numbness between the legs or loss of bladder or bowel control. Confusion, great thirst and constipation in someone with cancer may signal high calcium and also needs same-day care.
