Established scarring is largely permanent, but removing the cause, for instance stopping alcohol, curing hepatitis C or suppressing hepatitis B, can halt progression and sometimes allows partial improvement. When the liver can no longer cope, transplantation is the only treatment that replaces its function.

The main causes are long-term heavy alcohol use, chronic hepatitis B and C, and fatty liver disease linked to obesity and type 2 diabetes (now called metabolic dysfunction-associated steatotic liver disease). Less common causes include autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis, haemochromatosis, Wilson disease, alpha-1 antitrypsin deficiency and long-standing heart failure. Several causes often act together, for example alcohol plus excess weight.

Vomiting blood, black tarry stools, new confusion or severe drowsiness, fever with abdominal pain when you have ascites, a sharp fall in urine output or rapidly deepening jaundice are emergencies. Call emergency services or go to the nearest hospital; do not travel.

No. With cirrhosis of any cause the advice is to avoid alcohol completely.

Paracetamol at a reduced dose is generally preferred. Anti-inflammatory drugs such as ibuprofen can harm the kidneys and trigger bleeding. Check with your doctor.

When is transplant considered?

Typically after a first major complication, when the MELD score rises to around 15 or more, or for liver cancer within accepted criteria.
