Neck pain is felt between the base of the skull and the top of the shoulders. It comes most often from the muscles, small facet joints and discs of the cervical spine, and it commonly flares and settles over a lifetime. When a nerve root is pinched the pain runs down the arm, which is called cervical radiculopathy.
Most episodes improve within weeks. About one in ten people develop persistent pain lasting over 3 months, where posture, workload, stress and sleep keep it going as much as any structural change. A much less common but important problem is pressure on the spinal cord itself, called cervical myelopathy.
Symptome
- Aching and stiffness, worse after holding one position such as screen work or driving
- Reduced turning, noticed when reversing a car
- Pain spreading to the shoulder blade or back of the head
- Sharp or burning pain, tingling or numbness down one arm into specific fingers
- Waking with a locked, twisted neck (acute wry neck)
- Headache starting from the upper neck
- Clumsy hands or unsteady walking, which point towards the spinal cord

Everyday causes are muscle strain from sustained posture, sleeping awkwardly, and whiplash after a collision. With age, discs lose height and facet joints develop bony spurs (cervical spondylosis), seen on almost everyone's X-ray after 50 and often painless. A disc prolapse or spurs can narrow the exit for a nerve root, most often C6 or C7. Stress and poor sleep amplify pain. Less common causes include inflammatory arthritis, especially rheumatoid arthritis affecting the top of the neck, infection, fracture with osteoporosis, and tumours. Pain from the heart or the shoulder can also be felt in the neck.

- Clinical examination: Movement, tenderness, arm strength, reflexes, sensation and tests that reproduce arm pain. Walking and hand coordination screen for cord compression.
- MRI of the cervical spine: For arm pain lasting beyond 4 to 6 weeks, any weakness, or signs of myelopathy.
- X-rays: After injury, or with flexion and extension views to look for instability.
- Nerve conduction studies and EMG: Help separate a pinched neck nerve from carpal tunnel syndrome or ulnar nerve entrapment.
Behandlungsmöglichkeiten
- Keep it moving: Gentle range-of-movement exercise and normal activity. Soft collars are discouraged beyond a day or two.
- Physiotherapy: Strengthening of the deep neck and shoulder blade muscles, manual therapy and workstation advice have the best support for persistent pain.
- Medicines: Short courses of anti-inflammatories; nerve pain medicines such as amitriptyline or gabapentin for arm pain.
- Injections: Image-guided nerve root or facet joint injections in selected cases. Neck injections carry rare but serious risks and need an experienced operator.
- Surgery: Anterior discectomy with fusion or disc replacement, or a posterior decompression, for arm pain with weakness that fails 6 to 12 weeks of care, and for myelopathy, where the aim is to stop further deterioration.

Get urgent local care for neck pain after a fall, dive or road collision; with fever and a stiff neck; with new weakness, numb clumsy hands, difficulty walking or loss of bladder control; or with sudden severe neck pain and headache, dizziness, drooping eyelid or speech difficulty, which can indicate a tear in a neck artery.

Ordinary neck pain does not justify a flight. Travelling can be worthwhile for a surgical opinion on a proven disc prolapse or on cord compression, since these are planned operations with a short hospital stay of 1 to 3 nights and around 7 to 10 days before flying home. Bring your MRI on disc. If myelopathy is progressing quickly, have it treated where you are rather than delaying for travel arrangements.
