Every face is slightly asymmetrical, and mild differences between the two sides are normal and often unnoticed by others. Facial asymmetry becomes a medical or cosmetic concern when the difference in the jaw, chin, cheeks, eyes or soft tissue is clearly visible or affects the bite, chewing or eye closure.
Correction depends entirely on which layer is uneven: bone, teeth, muscle, fat or nerve function. Treatment can range from a small amount of filler to combined orthodontics and jaw surgery, so an accurate diagnosis comes before any plan.
Symptome
- A chin or lower jaw that points to one side
- A slanted smile line or a bite that does not meet evenly
- One cheek or jaw angle fuller or flatter than the other
- Eyes or eyebrows at different heights
- One side of the face that moves less or droops
- A difference that has appeared or worsened recently, which always needs medical review
Ursachen und Risikofaktoren
Causes fall into three groups. Developmental causes include uneven jaw growth such as condylar hyperplasia, hemifacial microsomia, cleft conditions, torticollis in infancy and, rarely, progressive loss of tissue on one side known as Parry-Romberg syndrome. Acquired causes include poorly healed facial fractures, jaw joint disease, removal of tumours, enlarged chewing muscle on one side from grinding, and ageing that affects the sides differently. Nerve causes include Bell's palsy and other facial nerve injuries. New asymmetry over days needs urgent assessment for stroke; over months it needs a check for a growing lump.
Wie es diagnostiziert wird
- Clinical examination and photographs: The surgeon measures the facial midline, the level of the eyes and mouth, and watches facial movement to separate bone, soft tissue and nerve causes.
- Dental and bite assessment: An orthodontist checks the dental midlines and any cant of the biting plane.
- 3D CT or cone beam CT: Shows the shape and size of each half of the jaw and cheekbones and is used for virtual surgical planning.
- Bone scan (SPECT): Used when one jaw joint may still be overgrowing, since surgery is timed for when growth has stopped.
- Nerve tests: Electromyography helps in long-standing facial palsy to decide between nerve and muscle procedures.
Behandlungsmöglichkeiten
- Fillers or fat grafting: Add volume to a flatter cheek, temple or jawline. Suitable for mild soft tissue differences; fat may need a second session.
- Botulinum toxin: Shrinks an enlarged masseter muscle on one side, or relaxes the overactive side in facial palsy to balance the smile.
- Genioplasty: The chin bone is cut and moved to the midline. Useful when the bite is even but the chin is off-centre.
- Orthognathic surgery with orthodontics: Repositions the upper and lower jaws when the bite and facial skeleton are both uneven. Treatment takes 18 to 24 months in total.
- Custom implants or bone contouring: Patient-specific implants build up a deficient jaw angle or cheekbone; bone can be reduced on a prominent side.
- Facial reanimation surgery: Nerve transfers or muscle transfers restore movement in lasting facial paralysis.
Wenn es dringend ist
Sudden drooping of one side of the face, especially with arm weakness, speech difficulty or confusion, may be a stroke: call your local emergency number at once. Facial weakness developing over a day or two, a rapidly growing swelling, or asymmetry after an injury also need prompt assessment close to home, not a planned trip.

A visit works well for assessment with 3D imaging and a clear plan, and for single-stage procedures such as genioplasty, implants or fat grafting, which need about 7 to 10 days. Jaw surgery is different: it requires months of orthodontic preparation and follow-up, so it suits you only if your orthodontist at home coordinates with the surgical team or you can make several visits. Children with growth-related asymmetry are best followed by a local craniofacial team over years.

Häufig gestellte Fragen
Is some asymmetry normal?
Yes. Measured differences of 2 to 3 millimetres are usual and generally invisible in daily life. Photographs and mirrors exaggerate them.
Can fillers fix a crooked jaw?
They can disguise mild unevenness. They cannot move a chin or correct a bite, and large volumes used to hide a bony problem tend to look heavy.
At what age can jaw surgery be done?
After facial growth ends, usually 17 to 18 in women and 18 to 21 in men. Operating earlier risks the asymmetry returning.
Will the result be perfectly symmetrical?
No surgeon can promise that. The realistic aim is a difference small enough that you and others stop noticing it.
