Fine needle aspiration uses a thin needle, finer than a blood test needle, to draw cells out of a lump so a pathologist can examine them under the microscope. It is the usual first test for thyroid nodules, enlarged lymph nodes, salivary gland lumps and some breast lumps.
It answers quickly whether a lump looks benign, suspicious or malignant. Because it collects loose cells and not a solid piece of tissue, it sometimes cannot give a definite answer.
What Fine Needle Aspiration Biopsy (FNA) shows
The pathologist looks at the shape and arrangement of individual cells. In the thyroid, results are reported with the Bethesda system in six categories from non-diagnostic to malignant, each with an estimated cancer risk and a recommended next step. In lymph nodes, FNA can show infection, reactive change or spread from a cancer elsewhere, although lymphoma usually needs a core or whole node biopsy for typing. Molecular tests can be run on the sample for indeterminate thyroid nodules, where available.

- Thyroid nodules that meet size and ultrasound criteria for sampling
- A neck lump lasting more than 3 weeks
- Enlarged lymph nodes in someone with known or suspected cancer
- Salivary gland swellings
- Cysts that can be drained and tested at once

You lie down with the neck slightly extended. The skin is cleaned and sometimes numbed. Under ultrasound guidance the doctor passes the needle into the lump and moves it gently for a few seconds; two to four passes are typical. You should not swallow or talk during each pass. It feels like pressure with a brief sting. The procedure takes 10 to 20 minutes and you go home after a few minutes of pressure on the site.

If a cytologist checks the slides on site you may hear the same day whether the sample is adequate. The formal report takes 2 to 5 working days, longer with molecular testing. Benign results lead to ultrasound follow-up; indeterminate results to a repeat FNA, molecular testing or diagnostic surgery; malignant results to surgical planning.

Can the needle spread cancer?
Seeding along the needle track is extremely rare with fine needles and is not a reason to avoid the test.
What does an indeterminate result mean?
The cells are neither clearly benign nor clearly malignant. In the thyroid this carries a cancer risk of roughly 10 to 40 percent depending on category, so the next step is individual.
