Hydrocephalus is a build-up of cerebrospinal fluid that enlarges the brain's ventricles. Surgery either diverts the fluid through a shunt, a thin tube with a valve running under the skin from the brain to the abdomen, or opens an internal bypass with an endoscope (endoscopic third ventriculostomy, ETV).
It relieves pressure and can reverse symptoms such as headache, walking difficulty and drowsiness. A shunt is a lifelong implant that can block or become infected, and about half of shunts placed in children need revision within some years.
What Hydrocephalus Surgery (Shunt and ETV) involves
MRI defines the type of hydrocephalus. For a ventriculoperitoneal shunt, a small hole is drilled in the skull, usually behind the right hairline or behind the ear, and a catheter passed into the ventricle, often with image guidance. It is connected to a valve placed under the scalp and to a long tube tunnelled under the skin of the neck and chest into the abdominal cavity through a small incision or laparoscope. Programmable valves can be adjusted later with a magnet from outside. For ETV, an endoscope is passed through a burr hole into the ventricle and a small opening is made in the floor of the third ventricle so fluid escapes to the surface of the brain; no hardware is left. In adults with suspected normal pressure hydrocephalus, a lumbar tap test or a few days of lumbar drainage first shows whether walking improves.
Who is a good candidate for Hydrocephalus Surgery (Shunt and ETV)?
The choice between shunt and ETV depends on age and on whether the fluid pathways are blocked.
- Obstructive hydrocephalus from aqueduct stenosis or a tumour blocking flow, often suited to ETV
- Normal pressure hydrocephalus in older adults with gait disturbance that improves after a tap test
- Hydrocephalus after brain haemorrhage, meningitis or head injury
- Infants with congenital hydrocephalus, in paediatric neurosurgical units
- Shunt malfunction needing planned revision
Es ist normalerweise nicht die richtige Wahl, wenn:
- Enlarged ventricles from brain shrinkage, which a shunt does not help
- Active infection of the spinal fluid or abdomen
- Rapidly worsening drowsiness or vomiting, which is an emergency for the nearest neurosurgical unit, not for travel
- Dementia without the typical gait pattern and with a negative tap test
Technikoptionen
- Ventriculoperitoneal (VP) shunt: The standard shunt; drains to the abdomen.
- Programmable and anti-siphon valves: Allow pressure settings to be changed without surgery; often chosen for normal pressure hydrocephalus.
- Endoscopic third ventriculostomy: For obstructive hydrocephalus; avoids an implant. Less successful in babies under 6 months.
- Ventriculoatrial or lumboperitoneal shunt: Alternatives when the abdomen is unsuitable or in selected communicating hydrocephalus.
Was passiert während Ihrer Behandlung
Shunt placement or ETV takes 1 to 1.5 hours under general anaesthesia. You stay 2 to 4 nights. There is mild headache and tenderness along the tube track. A scan checks catheter position and ventricle size before discharge.
Stitches come out in 7 to 10 days. Adults return to light activity in 2 to 3 weeks. In normal pressure hydrocephalus, walking improves first, often within days to weeks; memory and bladder control improve less reliably. Everyone with a shunt needs a named neurosurgical service near home, a record of the valve type and setting, and a valve check after any MRI if it is programmable.
- Zurück zur alltäglichen Aktivität: 2 bis 3 Wochen
- When results show: Days to weeks
- How long they last: Lifelong implant; revisions are common
Sicherheit, Risiken und Revisionspolitik
The operations are short, but long-term shunt problems are common and families must know the warning signs.
- Shunt blockage, causing return of headache, vomiting or drowsiness, which needs urgent assessment
- Shunt infection, mostly in the first 3 months, requiring removal and antibiotics
- Over-drainage with low pressure headaches or subdural collections
- Bleeding in the brain along the catheter path, uncommon
- Seizures
- ETV closure with recurrence of hydrocephalus, sometimes years later
- Abdominal complications such as fluid cysts or bowel injury, rare

Cost of Hydrocephalus Surgery (Shunt and ETV) in Türkiye
Clinic-Y does not publish a single price for Hydrocephalus Surgery (Shunt and ETV), because the honest figure depends on your case. What moves it:
- Type of valve, programmable ones costing more
- Shunt versus endoscopic procedure
- Use of neuronavigation or laparoscopic assistance
- Tap test or lumbar drainage trial beforehand
- Length of stay and imaging

Häufig gestellte Fragen
Can I fly with a shunt?
Yes. Cabin pressure does not affect shunt function. After surgery most surgeons advise waiting 1 to 2 weeks, and longer after ETV if air remains in the head on the scan.
Is it sensible to travel abroad for this?
Only for stable, planned cases with confirmed neurosurgical follow-up at home. Acute hydrocephalus and suspected shunt failure must be treated locally and immediately.
Can I have MRI scans with a shunt?
Yes, but programmable valves can be reset by the magnet and must be checked afterwards.
