Liver Blood Tests

Liver blood tests, often called liver function tests (LFTs), are a group of measurements from one blood sample: the enzymes ALT, AST, ALP and GGT, together with bilirubin, albumin and a clotting test (INR or prothrombin time).

They are a sensitive, low-cost screen for liver injury and bile duct problems. The name is misleading: enzymes show damage rather than function, results can be normal in established cirrhosis, and mild abnormalities are common and often harmless.

What Liver Blood Tests shows

ALT and AST rise when liver cells are inflamed or injured, as in fatty liver, viral hepatitis, alcohol-related damage, drug reactions or autoimmune disease. ALP and GGT rise when bile flow is obstructed by gallstones, bile duct disease or tumours; ALP also comes from bone, and GGT responds to alcohol and certain medicines. Bilirubin is the pigment responsible for jaundice. Albumin and INR reflect the liver's actual manufacturing capacity and fall in advanced disease. The pattern and ratios of these results direct the next tests, such as viral hepatitis serology, iron studies, autoantibodies, ultrasound and elastography.

When Liver Blood Tests is recommended

• Within a general health check
• Jaundice, dark urine, pale stools, itching or pain in the right upper abdomen
• Regular alcohol use, obesity or type 2 diabetes
• Before and during medicines that can affect the liver, such as isotretinoin, methotrexate or anti-tuberculosis drugs
• Monitoring of known hepatitis, fatty liver or cirrhosis
• Before surgery, including bariatric surgery

• Normal results do not rule out fatty liver, chronic hepatitis or cirrhosis
• They do not identify the cause, so further tests are needed
• A single mildly raised value is often temporary; repeating it is the first step
• Fibrosis is better assessed with elastography or composite scores such as FIB-4

• Fasting is not usually required unless lipids or glucose are being checked at the same time
• Avoid alcohol and very strenuous exercise for 1 to 2 days beforehand
• Tell the doctor about all medicines, supplements and herbal products, including bodybuilding and weight loss products

A nurse takes a small sample of blood from a vein at the elbow, which takes a couple of minutes. You can eat and carry on normally straight afterwards. The laboratory's automated analysers process the sample the same day.

Results are usually ready the same day or the next. Mild rises, below about twice the upper limit, are usually rechecked after 4 to 8 weeks with attention to alcohol, weight and medicines. Persistent or marked abnormalities lead to a liver screen and ultrasound. Jaundice together with high enzymes, or a raised INR, needs prompt specialist review.
