Neurodegenerative diseases are conditions in which particular groups of nerve cells gradually stop working and die. They include Alzheimer's disease and other dementias, Parkinson's disease, motor neurone disease (ALS), Huntington's disease and the atypical parkinsonian syndromes.
None can currently be reversed. Treatments ease symptoms, and a few newer drugs modestly slow certain diseases. An accurate diagnosis still matters, because treatable conditions can look similar.

Ursachen und Risikofaktoren
Most involve abnormal proteins, such as amyloid and tau, alpha-synuclein or TDP-43, building up in brain cells. Age is the strongest risk factor. A minority are directly inherited, Huntington's disease always and ALS or Alzheimer's in roughly 5 to 10 in 100 cases. Vascular risk factors, head injury, hearing loss and inactivity raise dementia risk.

- Specialist clinical assessment: Still the core of diagnosis, often repeated over time.
- MRI brain: Shows patterns of shrinkage and excludes tumour, stroke and hydrocephalus.
- Neuropsychological testing: Profiles memory, language and executive function.
- Blood tests: Exclude thyroid disease, B12 deficiency and other reversible causes.
- PET, DaT scan or CSF biomarkers: Support specific diagnoses such as Alzheimer's or Parkinson's disease.
- EMG: Required when motor neurone disease is suspected.
- Genetic testing: Offered with counselling when family history suggests it.

- Symptomatic medication: Levodopa for Parkinson's disease; cholinesterase inhibitors and memantine for Alzheimer's disease.
- Disease-slowing drugs: Riluzole for ALS; anti-amyloid antibodies for early Alzheimer's where licensed, with modest benefit and a risk of brain swelling or bleeding.
- Deep brain stimulation: For selected people with Parkinson's disease.
- Rehabilitation: Physiotherapy, speech and occupational therapy.
- Nutrition and breathing support: Feeding tubes and non-invasive ventilation in ALS.
- Care planning: Support for carers and advance decisions.

Sudden confusion, a rapid decline over days, new weakness or a fall with head injury is not normal progression and needs urgent local assessment for infection, stroke, bleeding or medication effects.
