Gallium-68 DOTATATE PET-CT is a whole-body scan for neuroendocrine tumours (NETs). The tracer binds to somatostatin receptors, which most well-differentiated NETs carry on their surface in large numbers. Closely related tracers, DOTATOC and DOTANOC, work in the same way.
It is far more sensitive than the older octreotide scan and than CT or MRI alone for finding the primary tumour and mapping spread, and it shows whether a patient is eligible for lutetium-177 DOTATATE therapy. Poorly differentiated, fast-growing neuroendocrine cancers often lose their receptors and may not show up, in which case FDG PET-CT is the more informative scan.
What Gallium-68 DOTATATE PET-CT shows
The scan reveals tumours of the small bowel, pancreas, lung, stomach, rectum and appendix, and their deposits in lymph nodes, liver and bone, often detecting lesions a few millimetres across that other imaging misses. It also images phaeochromocytoma, paraganglioma, medullary thyroid cancer and meningioma. Uptake intensity is compared with the liver and spleen (Krenning score); tumours brighter than liver are considered suitable for peptide receptor radionuclide therapy. Normal strong uptake is seen in the spleen, kidneys, adrenal and pituitary glands, and a part of the pancreas called the uncinate process, which can mimic a tumour and needs an experienced reader.
When Gallium-68 DOTATATE PET-CT is recommended
- Staging of a biopsy-proven neuroendocrine tumour before surgery
- Searching for an unknown primary when liver metastases or hormone syndromes (carcinoid flushing, insulinoma, gastrinoma) point to a NET
- Selecting patients for lutetium-177 DOTATATE therapy and assessing them afterwards
- Suspected recurrence with rising chromogranin A or 5-HIAA
- Phaeochromocytoma, paraganglioma and inherited syndromes such as MEN1, VHL or SDH mutations
Grenzen und Situationen, in denen ein anderer Test besser ist:
- High-grade (G3) or poorly differentiated neuroendocrine carcinoma, where FDG PET-CT is often better; some patients need both
- Insulinomas, of which up to half have few somatostatin receptors; exendin-based tracers or endoscopic ultrasound may be needed
- Pregnancy
- Inflammation, healing fractures and accessory spleens can take up tracer and cause false positives

- No fasting is required in most centres; drink water freely
- Long-acting octreotide or lanreotide: schedule the scan just before your next injection, usually in week 3 to 4 of the cycle
- Short-acting octreotide is usually paused for 24 hours, if your doctor agrees it is safe
- Bring pathology reports with Ki-67 and grade, previous scans and a list of injections with dates
- Book early, because gallium-68 is made on site from a generator in limited daily amounts
Was passiert während des Tests
The tracer is injected into an arm vein and you rest for 45 to 60 minutes. Scanning from the top of the head to the thighs takes 20 to 30 minutes on a quiet, open ring scanner. CT contrast may be added to define liver and pancreatic lesions. There are no sensations from the tracer. Allow about 2 hours in the department.

Reports are usually ready in 1 to 2 working days. They list the sites of disease and the intensity of uptake, and comment on suitability for lutetium therapy. The findings should be discussed in a neuroendocrine tumour board alongside histology and grade, since treatment ranges from surgery to somatostatin analogues, targeted drugs, PRRT and watchful waiting. Take the DICOM images home.

Availability of this scan is patchy in many countries, so travelling for it can be justified when your oncologist has requested it, particularly if you are being considered for lutetium-177 therapy in the same centre. How is it different from a normal PET-CT?
A standard PET-CT uses FDG, which tracks glucose use. DOTATATE tracks somatostatin receptors. Slow-growing NETs often use little glucose but are rich in receptors, so they show clearly with DOTATATE.
Do I have to stop lanreotide or octreotide?
Long-acting injections are not usually stopped. The scan is timed for the days just before the next dose. Follow your own doctor's advice.
Does a positive scan mean I can have lutetium therapy?
It is a necessary condition. Eligibility also depends on tumour grade, progression on current treatment, kidney function and blood counts.
Are DOTATOC and DOTANOC scans equivalent?
For practical purposes yes. They differ slightly in receptor subtype affinity, and results are interpreted in the same way.
