Apheresis is a family of treatments in which blood is passed through a machine that separates it into components, removes one of them and returns the rest. In plasmapheresis, or therapeutic plasma exchange, the liquid plasma is removed along with the harmful antibodies or proteins it contains and is replaced with albumin solution or donor plasma.
It works fast but only temporarily: it clears antibodies already in the blood without stopping the body making more. It is therefore almost always combined with immunosuppressive drugs. It is an established hospital treatment for specific diseases, not a wellness or anti-ageing therapy, for which there is no sound evidence.
What Plasmapheresis (Plasma Exchange) and Apheresis involves
Access is through two large arm veins or, more often for a full course, a temporary central line placed in a neck or groin vein under local anaesthetic. Blood flows into a centrifuge or membrane filter, plasma is separated and discarded, and the blood cells are returned mixed with replacement fluid. Citrate or heparin prevents clotting. One session exchanges about 1 to 1.5 times your plasma volume, around 3 to 4 litres. A course is typically 5 to 7 sessions over 10 to 14 days, with blood tests for clotting, calcium and antibody levels between sessions.
Who is a good candidate for Plasmapheresis (Plasma Exchange) and Apheresis?
Plasma exchange is used for defined indications agreed in international apheresis guidelines, usually in acutely ill inpatients.
- Anti-GBM (Goodpasture) disease
- Thrombotic thrombocytopenic purpura (TTP), where it is life-saving and urgent
- Guillain-Barre syndrome, myasthenic crisis and CIDP
- Antibody-mediated transplant rejection and desensitisation before incompatible transplants
- Hyperviscosity in Waldenstrom macroglobulinaemia; selected severe ANCA vasculitis
- LDL apheresis for severe familial hypercholesterolaemia not controlled by drugs
Es ist normalerweise nicht die richtige Wahl, wenn:
- People seeking detox, rejuvenation, long COVID or general wellness plasma exchange, which lack reliable evidence
- Unstable blood pressure or active bleeding
- Severe allergy to albumin or plasma products
- Conditions in which drugs alone work as well, which now includes many cases of ANCA vasculitis
- Emergencies such as TTP, which must be treated immediately in the nearest capable hospital and not by travel
Technikoptionen
- Centrifugal plasma exchange: Separates plasma by spinning; can run through arm veins at lower flow.
- Membrane plasma separation: Uses a filter on a dialysis-type machine; common in nephrology units and needs a central line.
- Double filtration plasmapheresis: A second filter removes large antibodies and returns albumin, reducing the need for replacement fluid.
- Immunoadsorption: Columns selectively bind antibodies; used in some centres for transplant desensitisation.
- LDL apheresis: Selective removal of LDL cholesterol every 1 to 2 weeks long term.
- Cell apheresis: Removal of white cells, platelets or red cells, or collection of stem cells for transplantation.
Was passiert während Ihrer Behandlung
Each session lasts 2 to 3 hours while you lie in a bed or recliner. You may feel tingling around the lips or fingers from citrate lowering calcium, which is treated with calcium during the session. Feeling cold, light-headed or tired is common. Pulse and blood pressure are monitored throughout. Central line insertion is done beforehand with local anaesthetic.
Vorbereitung auf Ihre Reise
Most courses are given to hospital inpatients at the start of a serious illness, so this is rarely a treatment to travel for. The realistic planned settings are desensitisation before a living related transplant or a scheduled course for a chronic neurological condition, needing 5 to 7 sessions on alternate days over about 2 weeks. Long-term LDL apheresis every 1 to 2 weeks must be arranged where you live.
You can usually walk around shortly after a session, but tiredness for the rest of the day is normal. Clotting factors and antibodies that protect against infection are lowered for a day or two, so invasive procedures are timed carefully. Benefit appears within days in conditions such as Guillain-Barre syndrome or myasthenia and may wear off after several weeks unless other treatment takes over. The central line is removed at the end of the course.
- Back to everyday activity: Rest on treatment days
- When results show: Days to 2 weeks, depending on the disease
- How long they last: Weeks, unless followed by other treatment
Sicherheit, Risiken und Revisionspolitik
Serious complications are uncommon in experienced units, but the procedure involves large vein access and blood products.
- Tingling, cramps or low blood pressure from citrate and fluid shifts
- Allergic reactions to donor plasma, occasionally severe
- Bleeding tendency because clotting factors are removed when albumin is the replacement
- Central line complications: infection, clot, bleeding or lung puncture
- Increased infection risk from loss of antibodies combined with immunosuppression
- Removal of some medicines from the blood, requiring dose timing adjustments
- Small risk of transfusion-transmitted infection when donor plasma is used

Cost of Plasmapheresis (Plasma Exchange) and Apheresis in Türkiye
Clinic-Y does not publish a single price for Plasmapheresis (Plasma Exchange) and Apheresis, because the honest figure depends on your case. What moves it:
- Anzahl der Sitzungen im Kurs
- Replacement fluid used: albumin, donor plasma or both
- Technique, as double filtration and immunoadsorption columns cost more
- Central line placement and inpatient versus day-case care
- Accompanying drugs such as rituximab or immunoglobulin

Häufig gestellte Fragen
Is plasmapheresis the same as dialysis?
No. Dialysis removes small waste molecules and water. Plasma exchange removes large proteins such as antibodies. The machines look similar and both may be run by nephrology units.
Is it the same as donating plasma?
The separation principle is the same, but therapeutic exchange removes far more plasma and replaces it.
Does plasma exchange slow ageing or clear toxins?
There is no good clinical evidence for this. It carries real risks and should be reserved for recognised medical indications.
Will I need it again?
That depends on the disease. Some conditions need a single course, others relapse, and a few need regular maintenance sessions.
