When a shoulder has dislocated more than once, the torn rim of cartilage (labrum) and stretched ligaments at the front of the socket no longer hold the ball in place. Stabilisation surgery reattaches them, or adds a block of bone when the socket edge has worn away.

For an arthroscopic Bankart repair you are positioned sitting up or on your side. Through three small portals, the surgeon frees the labrum, which has usually healed in the wrong place on the neck of the socket, and roughens the bone. Three to five small suture anchors are placed along the front rim and their threads are passed through the labrum and capsule, pulling the tissue up to recreate a bumper. A large dent in the back of the ball (Hill-Sachs lesion) can be filled by stitching the tendon behind into it, called remplissage.
