A kidney ultrasound uses sound waves to produce live images of the kidneys, ureters where visible, and bladder. It needs no radiation, no injection and no contrast dye, which makes it the first imaging test for almost every kidney question and safe in pregnancy and in kidney failure.
A Doppler examination can be added to show blood flow in the renal arteries and veins. When ultrasound cannot answer the question, CT or MRI follows: CT is better for stones and tumours, MRI for characterising cysts and masses without radiation.
What Kidney Ultrasound shows
The scan measures kidney length (normally about 10 to 12 cm in adults) and the thickness and brightness of kidney tissue. Small, bright kidneys indicate long-standing damage, while normal-sized kidneys suggest a more recent or potentially reversible problem. It detects hydronephrosis, meaning swelling from blocked urine drainage, as well as cysts and polycystic disease, solid masses, larger stones, scars, abscesses and congenital variants such as a single or horseshoe kidney. A bladder scan before and after passing urine shows incomplete emptying and an enlarged prostate. Doppler assesses renal artery narrowing, vein thrombosis and blood flow in a transplanted kidney.
When Kidney Ultrasound is recommended
- Reduced kidney function or a rising creatinine
- Blood or protein in the urine
- Flank pain or suspected stones, especially in pregnancy and in young people
- Recurrent or complicated urinary tract infections
- High blood pressure that is difficult to control or starts young
- Family history of polycystic kidney disease
- Follow-up of known cysts, hydronephrosis or a kidney transplant

- Small stones, particularly in the ureter, are often missed; low-dose CT is more accurate
- Cannot reliably tell a benign from a cancerous solid mass; contrast CT or MRI is required
- Image quality is reduced by obesity and bowel gas

- Doppler for renal artery stenosis depends heavily on the operator; CT or MR angiography is more definitive

- Usually no fasting is needed; some units ask for 4 to 6 hours without food to reduce bowel gas for Doppler studies
- Drink about half a litre of water an hour beforehand and avoid emptying your bladder if the bladder is to be examined

You lie on your back and then on each side while the sonographer or radiologist moves a gel-covered probe over your flanks and lower abdomen. You will be asked to hold a deep breath at times so the kidneys move below the ribs. It is not painful, though pressure over a tender kidney can be uncomfortable. The scan takes 15 to 20 minutes, or about 30 to 45 minutes with a full Doppler study. You may be asked to pass urine and have the bladder rescanned.

A radiologist's report is usually available the same day, often within an hour. Normal findings are reassuring but do not exclude early kidney disease, which is detected by blood and urine tests. Abnormal findings lead to the next step: CT for stones or masses, MRI for complex cysts, urology referral for obstruction, or a nephrology work-up, sometimes including biopsy, for bright or small kidneys.

I have a kidney cyst on my report. Is that serious?
Simple cysts are found in about a third of people over 50 and are harmless. Only cysts with thick walls, internal divisions or solid parts need further imaging.
Can ultrasound tell how well my kidneys work?
Not directly. It shows structure. Function is measured by blood creatinine, eGFR and urine tests.
Is it safe in pregnancy and for children?
Yes. It is the preferred kidney imaging method in both.
Why was I asked to come with a full bladder?
A full bladder allows its wall and the lower ureters to be seen, and lets the scanner measure how much urine remains after you empty it.
