Buttock augmentation adds volume using silicone gluteal implants, your own fat, or both. A buttock lift is a different operation that removes loose, sagging skin across the top of the buttocks, usually after major weight loss. They may be done separately or together.
Implants give a predictable, lasting increase in projection in slim people with no spare fat, and a lift corrects droop that no filler or exercise can. Implants enlarge mainly the upper and central buttock and not the hips, have a notable wound complication rate, and a lift leaves a long scar along the top of the buttocks.
What Buttock Augmentation and Lift involves
For implants, a single cut of about 6 cm is made in the crease between the buttocks. The surgeon creates a pocket within the gluteus maximus muscle (intramuscular), which hides the implant edges and keeps it away from the sciatic nerve, or less often under the muscle fascia. A solid, soft silicone implant, round or oval, is inserted on each side and the muscle, fascia and skin are closed in several layers, often over a drain. For a lift, a wedge of skin and fat is removed across the upper buttocks and lower back; the lower skin is pulled upward and anchored. Some surgeons keep part of this tissue, buried as a flap, to add fullness (auto-augmentation). Fat grafting may be added to the hips and outer buttock.
Who is a good candidate for Buttock Augmentation and Lift?
The right option depends on whether you lack volume, have excess skin, or both.
- You are slim with too little fat for a BBL and want more projection
- You have flat, sagging buttocks after major weight loss
- Your weight is stable and you do not smoke
- You can avoid sitting and sleeping on your back for 2 to 3 weeks
It is usually not the right choice if:
- You have enough fat and want wider hips, where fat transfer is more suitable
- You want very large implants; oversized implants cause wound breakdown and visibility
- You have diabetes that is poorly controlled or a weakened immune system
- Your job requires prolonged sitting that you cannot interrupt for some weeks
- You are considering injected silicone, biopolymers or large-volume fillers, which are dangerous and should be refused
Technique options
- Intramuscular gluteal implants: The most widely used plane; good cover and lower rates of visibility and displacement.
- Subfascial implants: Allows anatomical shapes in selected patients with thick tissue; edges may show more.
- Upper buttock lift: Removes loose skin through a scar at the top of the buttocks, often as part of a lower body lift.
- Auto-augmentation lift: Uses your own tissue flap for volume after weight loss.
- Composite augmentation: A moderate implant plus fat grafting to hips and edges for a smoother outline.
- Fat transfer (BBL): Covered on its own page; suits people with donor fat.
What happens during your treatment
Implant surgery takes 2 to 3 hours under general anaesthetic with 1 to 2 nights in hospital. You lie face down during the operation and wake on your front or side. Pain is significant for the first 3 to 5 days because the muscle has been split, and walking feels stiff. Drains often stay for several days.
Preparing for your trip
Plan for 12 to 14 days in Istanbul. Your surgeon reviews photographs and your medical history before you book, and you meet in person the day before surgery for examination, marking and consent with a professional interpreter.
- Stop smoking and nicotine at least four weeks before and after surgery; it is the single biggest avoidable cause of wound problems
- Tell the team about every medicine and supplement you take; blood thinners, some herbal products and hormone treatment may need to be paused on your doctor's advice
- Arrange for someone to travel with you or to be reachable, and keep the first days at home free of work and lifting
- Book a changeable return flight; your surgeon confirms when you are fit to fly
Recovery and results
No direct sitting for 2 to 3 weeks other than for the toilet, and sleep on your front. Short walks start on day one. Desk work with a cushion is possible at 3 weeks, driving at about 3 weeks, and gym work for the lower body at 8 to 12 weeks. The implants feel firm and high at first and soften over 3 to 6 months. The crease wound needs meticulous hygiene.
- Back to everyday activity: 3 weeks
- When results show: Shape at once; softens over 3 to 6 months
- How long they last: Implants are long-lasting but may need revision; lift results persist with stable weight
Safety, risks and revision policy
Gluteal implants have higher complication rates than breast implants, mostly because of the wound's position.
- Wound separation in the buttock crease, the most frequent problem
- Infection, sometimes requiring implant removal for several months
- Seroma around the implant
- Implant displacement, rotation or visible edges
- Capsular contracture and firmness
- Sciatic-type pain or numbness, usually temporary
- Asymmetry
- For lifts: long scars, wound healing problems and flattening if too much tissue is removed

Frequently Asked Questions
Implants or fat: how do I choose?
If you have enough fat, fat transfer shapes hips and buttocks together and avoids a foreign body. If you are lean, implants are the only way to add meaningful volume.
Can you feel the implants when sitting?
When placed within the muscle they are rarely felt. You sit mostly on the lower buttock and thigh bones, below where the implant lies.
Are buttock injections a shortcut?
No. Liquid silicone, biopolymers and unlicensed fillers cause chronic infection, hard lumps, skin death and deaths. Large volumes of hyaluronic acid are also unproven. Decline them.
How long do gluteal implants last?
They are solid elastomer rather than gel-filled, so they do not leak. Many stay in place for decades unless displacement, contracture or infection occurs.
