Lutetium-177 therapy is a targeted radioactive treatment given through a drip. A carrier molecule seeks out a marker on cancer cells and delivers a short-range dose of radiation to them. The lutetium itself is infused over 20 to 30 minutes. You then stay in a shielded room until radiation levels allow discharge, and a scan at 24 to 72 hours confirms where the drug has gone. Blood counts are checked every 2 to 3 weeks between cycles.

Who is a good candidate for Lutetium-177 Therapy (PSMA and DOTATATE Radioligand Therapy)?

It is for people with specific advanced cancers whose scans confirm the target is present.

• Metastatic castration-resistant prostate cancer that has progressed after hormone agents, usually after taxane chemotherapy too, with PSMA-positive disease
• Inoperable or metastatic well-differentiated neuroendocrine tumours of the gut or pancreas that have progressed on somatostatin analogues
• Adequate blood counts and kidney function
• Patients able to return for repeated cycles or to arrange them near home

Technique options

• Lu-177 PSMA-617: For prostate cancer. Typically up to 6 cycles, 6 weeks apart.
• Lu-177 DOTATATE (PRRT): For somatostatin receptor positive neuroendocrine tumours. Usually 4 cycles, about 8 weeks apart.
• Actinium-225 PSMA: An alpha emitter used in some centres when lutetium has stopped working. Evidence is more limited and dry mouth is a frequent problem.
• Dosimetry-guided treatment: Post-treatment scans are used to estimate the dose to tumours and kidneys and to adjust later cycles.

What happens during your treatment

The infusion visit lasts 4 to 6 hours. It is given without anaesthetic and is not painful beyond the cannula. Nausea can occur with the amino acid drip. Depending on local radiation rules you stay in a shielded room for some hours or 1 to 2 nights. You are asked to drink plenty and empty your bladder often.

Preparing for your trip

Plan 4 to 6 cycles spaced 6 to 8 weeks apart, each needing about 3 to 5 days in Istanbul. For about a week after each cycle you should limit close contact with children and pregnant women, and carry a letter for airport radiation detectors. Blood tests between cycles can be done at home and sent on.

Recovery and results

Most people feel tired for 1 to 2 weeks after each cycle. A dry mouth and mild nausea are common with PSMA therapy. Bone pain can flare briefly. Blood counts reach their lowest at 4 to 6 weeks. Response is assessed with PSA or chromogranin and imaging after 2 cycles and again at the end. Follow precautions for bodily fluids for several days, including sitting to urinate and flushing twice.

Safety, risks and revision policy

Side effects are usually milder than with chemotherapy, but they are real and some are late.

• Fatigue, nausea and reduced appetite for days after each cycle
• Falls in red cells, white cells or platelets, occasionally severe, more likely with heavy bone involvement
• Dry mouth and dry eyes with PSMA agents
• Gradual decline in kidney function
• Hormonal crisis with flushing and diarrhoea in functioning neuroendocrine tumours, which is uncommon
• A small long-term risk of myelodysplasia or leukaemia
• No response, or progression despite treatment
