MR-Guided Focused Ultrasound (MRgFUS) for Tremor

MR-guided focused ultrasound directs about a thousand beams of ultrasound through the intact skull to meet at a point a few millimetres wide deep in the brain. The combined energy heats and destroys that spot. For tremor, the target is a relay in the thalamus called the Vim nucleus, and the procedure is known as a focused ultrasound thalamotomy.

MR-guided focused ultrasound directs about a thousand beams of ultrasound through the intact skull to meet at a point a few millimetres wide deep in the brain. In essential tremor it reduces hand tremor on the treated side by roughly 50 to 70 percent on average, often visibly during the procedure, with no incision or implant. The lesion is permanent and cannot be adjusted afterwards, it is normally done on one side at a time, and tremor returns to some degree in a minority of patients over the years.

What MR-Guided Focused Ultrasound (MRgFUS) for Tremor involves

A movement disorder neurologist confirms the diagnosis and that medicines such as propranolol and primidone have failed or are not tolerated. A CT scan measures your skull density ratio, since thick or spongy bone can block the ultrasound. On the day your head is fully shaved, a frame is fixed to the skull under local anaesthetic, and a water-filled membrane is fitted around the scalp to cool it. You lie in the MRI scanner awake. Low-power test sonications warm the target briefly while the team asks you to draw spirals or hold a cup and checks for tingling, speech or balance changes. The position is fine-tuned, then the power is raised to about 55 to 60 degrees Celsius to make the permanent lesion.

Who is a good candidate for MR-Guided Focused Ultrasound (MRgFUS) for Tremor?

It is for disabling tremor that has not responded to medication.

It is usually not the right choice if:

What happens during your treatment

You are in the scanner for 2 to 4 hours, awake, with 10 to 20 sonications lasting 10 to 30 seconds each. During them you may feel head warmth or pressure, dizziness, nausea or a brief headache. Frame pins are the most uncomfortable part. Most people stay one night and have an MRI the next day to confirm the lesion.

Preparing for your trip

One treatment session. Allow 2 to 3 days beforehand for neurological assessment and the CT skull measurement (ideally sent in advance so you do not travel in vain), then 3 to 5 days afterwards while early unsteadiness settles. A week to 10 days in total is typical.

Recovery and results

Tremor relief is immediate. Unsteadiness, tingling of the lips or fingertips, and mild slurring are common in the first days as swelling forms around the lesion, and mostly fade within 1 to 3 months. Take care on stairs and do not drive until your balance is reliable. Hair regrows. Follow-up is with your neurologist at home at about 3 months and then yearly.

Safety, risks and revision policy

There is no wound, so infection and hardware problems do not arise, but a brain lesion has its own risks.

Every written proposal arranged through Clinic-Y states what the clinic covers if a correction is needed. Ask for it before you book, not after.

Frequently Asked Questions

Can both hands be treated?

Standard practice treats one side. A second side may be offered after 9 to 12 months in centres with experience, accepting a higher risk to speech and balance.

How do I know if my skull is suitable?

A CT scan gives a skull density ratio. Send the CT before booking travel; values below about 0.40 to 0.45 make success less likely.

How does it compare with deep brain stimulation?

DBS can be adjusted, treats both sides and gives somewhat greater tremor control, but involves surgery and a lifelong device. Focused ultrasound is a one-off with no implant, and it is irreversible.

Does it treat Parkinson's disease?

It can reduce Parkinsonian tremor on one side. It does not slow the disease or help with other symptoms.
