Aesthetic Scalp Surgery

Aesthetic scalp surgery is a group of operations on the hair-bearing skin of the head: revising wide or bald scars, removing bald patches by advancing neighbouring hairy scalp, stretching scalp with tissue expanders, and lowering a high hairline. It is different from a hair transplant, which moves individual follicles rather than skin.

It can remove or shrink a fixed bald area, such as a burn scar, an old injury or a scar from earlier strip hair surgery, in a way grafts alone may not. Its limit is scalp tightness: only so much skin can be moved, scars can stretch again, and it does nothing to stop ongoing pattern hair loss.

What Aesthetic Scalp Surgery involves

The surgeon measures the bald or scarred area and tests how loose your scalp is by pinching and sliding it. For a small scar, the scar is cut out, the scalp on both sides is freed from the skull lining and the edges are closed in layers, often with a bevelled (trichophytic) cut so hair grows through the new scar. For a larger patch, a silicone balloon expander is placed under nearby hair-bearing scalp and filled with saline weekly for 6 to 12 weeks; at a second operation the stretched skin is advanced over the defect. Rotation flaps are an alternative. Hairline lowering removes a strip of forehead skin and advances the scalp forward. Hair grafts may be added months later to soften any scar.

Who is a good candidate for Aesthetic Scalp Surgery?

It suits people with a stable, well-defined bald or scarred area and enough loose, healthy scalp nearby.

• You have a scar from injury, burns, surgery or an old strip hair transplant that shows through your hair
• You have a patch of scarring alopecia that a specialist confirms is inactive
• Your scalp is mobile enough on examination
• You accept a staged plan and, with expanders, a visibly swollen head for some weeks

It is usually not the right choice if:

• You have progressive male or female pattern hair loss as the main problem; medication and transplant suit this better
• Your scarring alopecia is still active, as new surgery can trigger further loss
• Your scalp is very tight or has been irradiated
• You smoke heavily, which threatens flap blood supply
• You cannot stay or return for the second stage of an expander plan

With expanders, plan for weekly fills and a growing bulge that is hard to hide for 2 to 3 months.
