Electrolyte Disorders

Electrolytes are minerals dissolved in the blood, mainly sodium, potassium, calcium, magnesium and phosphate. They control nerve and muscle function, heart rhythm and the balance of water in the body. An electrolyte disorder means one or more of them is too high or too low.

The kidneys do most of the fine tuning, so these problems are common in kidney disease, but tablets such as diuretics, vomiting, diarrhoea and hormone conditions are equally frequent causes. The abnormal number is a clue; the real task is to find out why it happened.

Common causes include diuretics and other medicines (ACE inhibitors, some antidepressants, proton pump inhibitors, laxatives), fluid loss from vomiting, diarrhoea or sweating, acute or chronic kidney disease, heart failure and liver cirrhosis, uncontrolled diabetes, and hormone disorders of the adrenal, parathyroid, thyroid or pituitary glands. Low magnesium often hides behind low potassium or low calcium that will not correct. High phosphate is mostly a feature of advanced kidney disease; low phosphate follows poor nutrition, alcohol excess or refeeding after starvation.

When it is urgent

Palpitations, fainting, severe weakness, new confusion, seizures or repeated vomiting need emergency assessment locally. Very high potassium and very low sodium can be fatal and are treated within hours, not planned around travel.

Acute disturbances must be corrected where you are, and flying with an unstable potassium or sodium is unwise. A planned visit is reasonable for an unexplained, recurring problem: a nephrology and endocrine work-up with blood, urine and hormone tests can usually be completed in a few days. Long-term monitoring then continues at home.

For healthy people exercising hard in heat they help replace sweat losses. They do not treat a medical electrolyte disorder and the potassium in some of them can be harmful in kidney disease.

A single mildly abnormal result is often due to the way the sample was taken or stored. It should be repeated before any conclusions are drawn.

Why is sodium corrected slowly?

Brain cells adapt to low sodium. Raising it too fast can cause permanent nerve damage, so doctors limit the rise per day.
