Gastric sleeve surgery (sleeve gastrectomy) removes about 75 to 80 in 100 of the stomach, leaving a narrow tube the shape of a banana. You feel full after small portions, and levels of the hunger hormone ghrelin fall.
Typical loss is 55 to 70 in 100 of excess weight over 12 to 18 months, with improvement in diabetes, blood pressure and sleep apnoea. It cannot be reversed, it may cause or worsen acid reflux, and weight can return if eating habits drift.
What Gastric Sleeve involves
The operation is laparoscopic, through four or five small incisions. The surgeon frees the outer curve of the stomach from its blood supply and from the spleen. A sizing tube (bougie) is passed through the mouth to lie along the inner curve. A stapler is fired repeatedly alongside this tube from near the outlet of the stomach up to the angle beside the oesophagus, dividing and sealing the stomach. The detached portion, about three quarters of the stomach, is removed through one of the incisions. The staple line may be reinforced with stitches or buttress material, and it is tested for leaks. The intestines are not rerouted.
Who is a good candidate for Gastric Sleeve?
It is for adults with obesity that has not responded to supervised non-surgical treatment, following a team assessment.
- BMI of 40 or more, or 35 or more with weight-related illness
- BMI of 30 to 35 with poorly controlled type 2 diabetes, in selected cases
- People who prefer to avoid intestinal rerouting and its nutritional burden
- Very high BMI, as a first stage before a second operation
It is usually not the right choice if:
- Severe acid reflux, Barrett's oesophagus or a large hiatus hernia, where bypass is usually the better choice
- BMI under 30, or wanting surgery for cosmetic weight loss
- Untreated eating disorder, substance dependence or unstable psychiatric illness
- Pregnancy or plans for pregnancy within 12 to 18 months
- People unfit for general anaesthesia
- Anyone without access to follow-up and blood tests at home

Blood tests are advised at 3, 6 and 12 months and then yearly. Hair thinning at 3 to 6 months is common and temporary.
