Cataract surgery removes the cloudy natural lens of your eye and replaces it with a clear artificial lens, called an intraocular lens or IOL. It is the only treatment that clears a cataract; drops and glasses cannot.

It restores the clarity that the cataract took away and, depending on the lens chosen, can also reduce your need for glasses. It cannot repair damage from other eye disease, so vision stays limited if you also have macular degeneration, advanced glaucoma or diabetic retinal damage.

What Cataract Surgery involves

Before surgery your eye is measured with optical biometry to calculate the lens power, and the retina is checked so that expectations are realistic. On the day, drops widen the pupil and numb the eye. The surgeon makes a cut of about 2 to 3 mm at the edge of the cornea, opens the front of the lens capsule in a circle, and breaks up the cloudy lens with an ultrasound probe (phacoemulsification), suctioning the pieces out. The folded artificial lens is injected through the same cut and unfolds inside the capsule. The wound usually seals itself without stitches. An antibiotic is placed in the eye and a clear shield is taped on. Eyes are operated one at a time, typically a few days to two weeks apart.

Who is a good candidate for Cataract Surgery?

Surgery is worth considering when the cataract, rather than your glasses prescription, is what limits daily life.

• Blurred, dim or glare-affected vision that interferes with driving, reading or work
• A cataract confirmed on examination as the main cause of the visual loss
• A cataract that prevents your doctor from monitoring or treating the retina
• People who accept that glasses may still be needed for some distances

It is usually not the right choice if:

• A mild cataract that does not yet bother you; there is no benefit in operating early
• Active eye infection or uncontrolled inflammation inside the eye
• Poorly controlled diabetes or blood pressure until it is stabilised
• Anyone expecting surgery to fix vision lost to retinal or optic nerve disease

• Phacoemulsification: The standard ultrasound technique used for the great majority of cataracts.
• Femtosecond laser-assisted surgery: A laser makes the incisions and softens the lens. Outcomes are similar to standard surgery for most eyes; it may help in selected cases.
• Monofocal lens: Gives clear vision at one distance, usually far. Reading glasses are expected. Lowest risk of glare and haloes.
• Toric lens: Corrects significant corneal astigmatism at the same time.
• Extended depth of focus or trifocal lens: Reduces dependence on glasses but can cause haloes and slightly lower contrast. Not suited to eyes with other disease.

The operation takes about 15 to 30 minutes per eye under anaesthetic drops, sometimes with light sedation. You are awake and see bright lights and colours but no detail. You feel water running and mild pressure. You need to lie flat and still. You go home the same day with a shield over the eye and should not drive.

• Stop smoking and nicotine at least four weeks before and after surgery; it is the single biggest avoidable cause of wound problems
• Tell the team about every medicine and supplement you take; blood thinners, some herbal products and hormone treatment may need to be paused on your doctor's advice
• Arrange for someone to travel with you or to be reachable, and keep the first days at home free of work and lifting
• Book a changeable return flight; your surgeon confirms when you are fit to fly

Vision is often hazy on the first day and clears over several days. You use antibiotic and anti-inflammatory drops for about 4 weeks. Avoid rubbing the eye, swimming, eye make-up and heavy lifting for 2 weeks, and wear the shield at night for the first week. Most desk work is possible within a few days. A final glasses test is done at 4 to 6 weeks, once the eye has settled.

Cataract surgery has a high success rate, but it is still an operation inside the eye and serious problems, though uncommon, can affect sight.

• Infection inside the eye (endophthalmitis), rare but sight-threatening and needing emergency treatment
• Tear of the lens capsule during surgery, which may require a further procedure or a different lens position
• Swelling of the macula causing blurred central vision for weeks, usually treated with drops
• Clouding of the capsule behind the lens months or years later, cleared with a short YAG laser treatment
• Retinal detachment, more likely in very short-sighted eyes
• A residual prescription, so glasses or a laser enhancement may still be needed
• Haloes, glare or reduced contrast, mainly with multifocal lenses
