All-on-4 dental implants

All-on-4 is a way of replacing every tooth in one jaw with a fixed bridge supported by four implants. Two implants go in upright at the front and two are tilted at the back to use the denser front bone and avoid the sinus or the lower jaw nerve.

It gives people with no teeth, or with teeth that cannot be saved, a fixed set that is not taken out at night. The limit is that everything rests on four supports: if one fails the whole bridge is affected, and the bridge covers the gum so cleaning underneath takes daily effort.

What All-on-4 dental implants involves

After a cone beam CT scan and a design of the new bite, any remaining teeth in that jaw are removed and the bone ridge is levelled so the bridge has room. Four implants are placed, the back pair angled at up to about 45 degrees. Angled connectors called multi-unit abutments are attached, the gum is stitched and impressions or scans are taken. Within 1 to 3 days a lightweight acrylic temporary bridge is screwed onto the implants. After 3 to 6 months of healing you return for the final bridge, usually a milled titanium or zirconia frame, which takes several fitting visits over about a week.

Who is a good candidate for All-on-4 dental implants?

It is meant for a whole jaw, not for a few gaps. It suits adults who have lost, or are about to lose, all teeth in the upper or lower arch.

• You wear a full denture that moves and want fixed teeth
• Your remaining teeth are loose from advanced gum disease and cannot be kept
• You have limited bone at the back and want to avoid a sinus lift
• You can eat a soft diet for the whole healing period
• You can return for the final bridge after several months

It is usually not the right choice if:

• People who still have sound, restorable teeth: removing healthy teeth for this is rarely justified
• Heavy smokers and people with uncontrolled diabetes
• Severe night-time grinders, unless a guard is accepted and the design is strengthened
• Those with very thin bone at the front of the jaw too
• Anyone unable to clean under a fixed bridge with brushes or a water flosser
