Back pain is pain felt anywhere from the bottom of the ribs to the buttock crease. The outlook is better than it feels at the time. Most acute episodes ease substantially within a few weeks. Causes and risk factors

Common mechanical causes are muscle and ligament strain, disc degeneration, a disc prolapse pressing on a nerve, worn facet joints, spinal stenosis and spondylolisthesis. Risk rises with heavy or repetitive work, long sitting, smoking, excess weight, low fitness, poor sleep and low mood. A small minority have a specific disease: an osteoporotic fracture, inflammatory arthritis such as ankylosing spondylitis (typically under 40, with stiffness that improves on exercise), infection, or cancer in the bone. Pain can also be referred from the kidneys, aorta, pancreas or pelvis.

Go to a local emergency department for numbness in the saddle area, new bladder or bowel problems, weakness in both legs, or a foot that suddenly drops. Also seek prompt care for back pain with fever, after a significant fall, with unexplained weight loss or a history of cancer, or sudden tearing pain with faintness, which can indicate an aortic problem.

Travelling to Türkiye for treatment

A trip does not suit an acute flare, and long flights are miserable with one. It can suit someone with months of pain who wants a complete assessment in a few days: examination, MRI, a spinal surgeon and a rehabilitation physician giving a joint opinion, and if appropriate an injection or a planned operation. An intensive 1 to 2 week rehabilitation block can teach a programme, but the benefit comes from continuing it at home for months.

Do I need a scan?

Usually not in the first 6 weeks unless there are warning signs. Early scanning has not been shown to improve outcomes and often finds age-related changes that cause worry without explaining the pain.

Is my disc bulge the cause?

Perhaps not. Bulges appear in a large share of adults with no pain. A bulge matters when it matches the leg and the nerve affected.

Will I end up needing surgery?

Few people do. Even with a proven disc prolapse, most sciatica settles within 3 months without an operation.

What helps chronic pain the most?

Regular exercise you can sustain, good sleep, a healthy weight, stopping smoking, and support for mood. No single injection or device replaces these.
