Limited Mouth Opening (Trismus)

Trismus means being unable to open the mouth fully. Normal opening is about 40 to 50 mm, roughly three fingers' width between the front teeth. Opening below about 35 mm is considered restricted.

It is a sign with many causes, from a temporary muscle spasm after dental work to scarring after radiotherapy or a fused jaw joint. It interferes with eating, speaking, tooth cleaning and dental treatment. Finding the cause matters, because a few causes are serious and many respond well to early stretching.

Symptoms

• Mouth opening limited to one or two fingers' width
• Tightness or pain in the cheek, temple or in front of the ear
• Jaw deviating to one side on opening
• Difficulty biting into larger foods
• Trouble brushing the back teeth
• Clicking followed by a locked joint
• Altered speech

Causes and risk factors

Short-term causes include infection around a wisdom tooth or a deeper dental abscess, muscle irritation after an anaesthetic injection, a long dental appointment or wisdom tooth removal, jaw fracture and tonsil abscess. Jaw joint causes include a displaced disc that blocks opening, arthritis and ankylosis, where the joint fuses after injury or childhood infection. Long-term scarring follows radiotherapy for head and neck cancer, surgery, burns and oral submucous fibrosis, which is linked to chewing areca (betel) nut. Tumours of the mouth, throat or jaw can present this way, and tetanus is a rare but classic cause.

How it is diagnosed

• Measurement of opening: The gap between the incisors is recorded in millimetres to track progress.
• Examination of mouth, throat, muscles and joints: Looks for infection, scarring, tenderness and any suspicious lump or ulcer.
• Panoramic X-ray: Shows wisdom teeth, fractures and the shape of the joints.
• CT scan: Defines bony fusion of the joint and deep infections.
• MRI of the jaw joints: Shows disc position and soft tissue, and helps exclude tumour.
• Biopsy: For fibrous bands of submucous fibrosis or any unexplained lesion.

Treatment options

• Treat infection: Drainage, antibiotics and dealing with the offending tooth.
• Rest, warmth and anti-inflammatory medicine: For muscle spasm after dental treatment, which usually settles in 1 to 2 weeks.
• Stretching exercises and jaw-opening devices: Stacked tongue depressors or devices such as TheraBite, used several times daily for months. The mainstay for radiation and scar-related trismus.
• Physiotherapy and splints: For jaw joint and muscle disorders.
• Arthrocentesis or arthroscopy: Washing out the joint frees a locked disc in many cases.
• Surgery for ankylosis: Release of the fused joint (gap arthroplasty) or total joint replacement, followed by intensive exercises to prevent re-fusion.
• Surgical release of scar bands: With grafts or flaps for advanced submucous fibrosis, after stopping areca nut.

When it is urgent

Seek emergency care locally for limited opening with fever, swelling under the jaw or in the neck, difficulty swallowing, drooling or noisy breathing. Jaw stiffness with muscle spasms after a wound also needs immediate assessment. Progressive painless trismus should be examined within two weeks to exclude a tumour.

Travelling to Türkiye for treatment

Trismus from infection or a recent injury should be treated at home straight away. Travel can suit planned surgery for jaw joint ankylosis or joint replacement, arthroscopy, or release of scarring, typically with a stay of 7 to 14 days.
