Peripheral Neuropathy

Peripheral neuropathy is damage to the nerves that run from the spinal cord to the limbs and organs. It most often affects the longest nerves first, so symptoms typically begin in the toes and feet and may later reach the hands.

It is a finding with many possible causes rather than a single disease. Treatment works on the cause where one can be found and on pain and safety where it cannot. Nerves recover slowly, and some numbness is often permanent.

Symptoms

• Numbness, tingling or a feeling of wearing socks when barefoot
• Burning, stabbing or electric pain, often worse at night
• Unsteadiness, especially in the dark
• Weakness in the feet or ankles, tripping or foot drop
• Cuts or blisters on the feet that you do not feel
• Cramps or twitching in the calves
• Dizziness on standing, bowel, bladder or sexual problems when the autonomic nerves are involved

Causes and risk factors

Diabetes is the commonest cause worldwide, and even pre-diabetes can be responsible. Other frequent causes are heavy alcohol use, vitamin B12 deficiency, chemotherapy drugs, kidney failure, an underactive thyroid and abnormal blood proteins. Less often the cause is an immune condition such as Guillain-Barre syndrome or CIDP, vasculitis, an infection such as HIV or leprosy, an inherited neuropathy such as Charcot-Marie-Tooth disease, or amyloidosis. In roughly a quarter of people, mostly older adults with mild sensory symptoms, no cause is found despite full testing.

How it is diagnosed

• Examination: Testing sensation, reflexes, strength and walking shows the pattern, which narrows down the likely causes.
• Blood tests: Glucose and HbA1c, vitamin B12, kidney, liver and thyroid function, and protein electrophoresis cover the common treatable causes.
• Nerve conduction studies and EMG: Measure how well signals travel along nerves and show whether the damage affects the nerve fibre or its insulating sheath, which changes treatment.
• Further tests in selected cases: Lumbar puncture, genetic testing, skin biopsy for small fibre neuropathy or, rarely, nerve biopsy when vasculitis or amyloid is suspected.

Treatment options

• Treating the cause: Good glucose control, stopping alcohol, replacing B12 or changing a responsible medicine can halt progression and allow partial recovery.
• Medicines for nerve pain: Duloxetine, amitriptyline, gabapentin or pregabalin. They reduce pain in many people but rarely remove it, and ordinary painkillers usually do little.
• Immune treatments: Immunoglobulin, steroids or plasma exchange for inflammatory neuropathies such as CIDP, under a neurologist's supervision.
• Foot care: Daily inspection, well-fitting shoes and podiatry to prevent ulcers, which matter most in diabetes.
• Physiotherapy and orthoses: Balance training, strengthening and ankle-foot splints for foot drop reduce falls.

When it is urgent

Weakness that spreads upwards over days, particularly with difficulty breathing, swallowing or facial weakness, may be Guillain-Barre syndrome and needs emergency hospital care. Sudden loss of bladder or bowel control with numbness between the legs points to spinal cord or nerve root compression and is also an emergency. An infected foot ulcer with spreading redness or fever needs same-day treatment.

Travelling to Türkiye for treatment

Most neuropathies are long-term conditions managed by your own doctor. A trip is reasonable if you have not been able to get a full work-up, since blood tests, nerve conduction studies and a neurologist's review can usually be completed in 3 to 5 days. It is not a sensible reason to travel if you need repeated immunoglobulin infusions or ongoing diabetic care. Be wary of offers of stem cells, ozone or vitamin infusions to regenerate nerves, which are not supported by good evidence.
