Blepharoplasty is surgery on the eyelids. Upper lid surgery removes the fold of surplus skin that sits on or over the lashes; lower lid surgery deals with under-eye bags caused by bulging fat and, where needed, loose skin.
It makes the eyes look less tired and, when heavy upper lids block the top of your vision, can widen your visual field. It does not lift a low eyebrow, erase crow's feet or dark circles caused by pigment, and removing too much can make it hard to close the eye.
What Blepharoplasty (Eyelid Surgery) involves
For the upper lids, the surgeon marks the natural crease with you sitting up, measures how much skin can go while leaving enough to close the eye, and removes a crescent of skin with a strip of muscle if required. A small fat pad near the nose may be trimmed. The cut is closed with a fine running stitch hidden in the crease. For the lower lids, fat is reached through the inside of the lid (transconjunctival), leaving no skin scar, or through a cut just under the lashes when skin must also be tightened. Modern practice often repositions fat into the tear trough rather than removing it, to avoid a hollow look. The outer corner may be supported with a canthopexy stitch.

It helps most when the problem is true excess skin or fat of the lids themselves.

- Your brow has dropped and is the real cause of the hooding; a brow lift may be needed

- Your puffiness comes and goes with allergy, salt or sleep
- You have uncontrolled blood pressure or take blood thinners that cannot be paused

Scars fade into the crease over 2 to 3 months.

- Asymmetry between the eyes
- A hollow look after over-removal of fat
- Visible or lumpy scars, small white cysts along the stitch line
- Bleeding behind the eye, a rare emergency that can threaten sight
