Brazilian Butt Lift (BBL)

A Brazilian butt lift (BBL) enlarges and reshapes the buttocks with your own fat. Fat is taken by liposuction from the waist, back, abdomen or thighs, purified and injected into the buttocks and hips. Despite the name it does not lift loose skin.

It changes the waist-to-hip ratio in one operation, using no implant. Part of the transferred fat is absorbed, so final size is smaller than it looks at first, and the procedure has a history of rare but fatal fat embolism when fat is injected into or under the gluteal muscle. Safe technique is the central issue.

What Brazilian Butt Lift (BBL) involves

You are marked standing. Under general anaesthetic, liposuction is performed around the trunk, which both supplies fat and narrows the waist. The fat is washed or filtered to remove fluid, oil and blood. It is then injected through small cuts using blunt cannulas in many thin passes, strictly into the layer between skin and muscle. Current safety guidance asks for injection above the muscle fascia only, a cannula of at least 4 mm angled away from the deep veins, and increasingly real-time ultrasound to confirm cannula position. Volumes commonly range from 300 to 800 ml per side depending on your frame and skin. The small cuts are closed and a garment that compresses the liposuction areas but not the buttocks is fitted.

Who is a good candidate for Brazilian Butt Lift (BBL)?

You need enough spare fat, good skin tone and realistic size goals.

• You have enough donor fat at the waist, back or abdomen
• You want a moderate increase and better hip and waist shape
• Your BMI is roughly between 22 and 30
• You can avoid sitting directly on your buttocks for several weeks
• You do not smoke

It is usually not the right choice if:

• You are very slim with little fat to harvest; implants may be discussed instead
• You want a very large increase in one session, which pushes unsafe volumes
• Your main problem is sagging skin after weight loss, which needs a buttock lift
• You have a history of blood clots or serious heart or lung disease
• You plan a long-haul flight within days of surgery

Surgery takes 2.5 to 4 hours under general anaesthetic, with 1 night in hospital. You wake lying on your front or side. The liposuction areas ache and leak fluid for a day or two; the buttocks feel tight and swollen rather than painful.

Desk work is possible at 2 weeks with a cushion, exercise at 6 to 8 weeks. Roughly 50 to 70 per cent of grafted fat survives; the volume you have at 3 to 6 months is what stays.

• Fat embolism to the lungs, rare but potentially fatal
• Deep vein thrombosis and pulmonary embolism, made more likely by long flights
• Fat necrosis: firm lumps or oily cysts
• Infection, including deep abscess
• Asymmetry or uneven fat survival
• Contour irregularities and loose skin at liposuction sites
• Seroma and prolonged swelling
• Less volume than hoped after absorption
