Sinus Lift (Crestal and Lateral Window)

A sinus lift raises the floor of the maxillary sinus above your upper back teeth and places bone graft beneath it, creating enough bone height to hold dental implants. In the crestal (internal) approach this is done through the implant drill hole itself; in the lateral approach a small window is opened in the side of the jaw.

It makes implants possible where bone has shrunk to a few millimetres after tooth loss. It adds healing time, and success depends on a healthy sinus and on not smoking.

What Sinus Lift (Crestal and Lateral Window) involves

A cone beam CT scan measures the remaining bone height and checks the sinus for disease or dividing walls. For a crestal lift, used when about 5 to 8 mm of bone remains, the implant site is drilled to just short of the sinus floor, and the floor is then gently tapped or pushed upwards with osteotomes, a balloon or a hydraulic kit, lifting the sinus membrane intact by 2 to 4 mm. Graft material is packed through the hole and the implant is placed at once. For a lateral window lift, used when less than about 4 to 5 mm remains, a gum flap is raised, an oval window is outlined in the thin outer wall with a piezoelectric tip, the membrane is peeled up carefully, and the space is filled with graft and covered with a collagen membrane before the gum is stitched.

Who is a good candidate for Sinus Lift (Crestal and Lateral Window)?

Your CT scan decides whether you need a lift at all and which kind.

• Missing upper premolars or molars with too little bone height under the sinus
• Healthy sinuses without chronic infection
• Non-smokers or those willing to stop around surgery
• People who want fixed implant teeth instead of a removable denture
• Remaining bone of 5 mm or more for the simpler crestal approach

It is usually not the right choice if:

• Active sinusitis, large polyps or cysts, until treated by an ENT specialist
• Heavy smokers, who have clearly higher failure rates
• Uncontrolled diabetes, or recent radiotherapy to the upper jaw
• People taking intravenous bisphosphonates
• Cases where short implants, tilted implants or zygomatic implants would avoid grafting altogether

Expect swelling of the cheek and perhaps bruising for 3 to 7 days after a lateral lift, far less after a crestal lift. For 2 weeks do not blow your nose, sneeze with your mouth open, avoid straws, diving and playing wind instruments. Short flights after a few days are normally accepted, but ask your surgeon; decongestants are often prescribed. Graft bone matures in 4 to 9 months before implants are loaded or placed.

The main intra-operative risk is tearing the thin sinus membrane.

• Membrane perforation in roughly 10 to 25 in 100 lateral lifts; small tears are patched, large ones may mean postponing the graft
• Sinus infection, sometimes needing graft removal
• Graft failure or insufficient bone formation
• Nosebleed and a blocked feeling for some days
• An opening between mouth and sinus that fails to heal, which is rare
• Implant failure, which is more likely in smokers
