A foraminotomy widens the small bony tunnel (foramen) where a spinal nerve root leaves the spine. Bone spurs, thickened ligament or a bulging disc can narrow this tunnel and pinch the nerve, causing arm pain from the neck or leg pain from the lower back.
It relieves nerve pain, tingling and weakness in the limb in most well selected patients. It is much less reliable for neck or back ache itself, and arthritis elsewhere in the spine continues.
What Foraminotomy involves
MRI, and sometimes CT, confirm which foramen is narrow and that it matches your symptoms. You lie face down. Through a 2 to 3 centimetre incision over the affected level, or through a tube or endoscope of under a centimetre, the surgeon moves muscle aside and uses a fine drill and small punches to remove part of the facet joint edge and the overgrown bone and ligament roofing the nerve. Less than half of the facet joint is removed so the spine stays stable and no fusion is needed. Any loose disc fragment under the nerve is taken out. The nerve is checked to be free along its course, bleeding is controlled and the wound is closed in layers. In the neck this is called a posterior cervical foraminotomy.
Who is a good candidate for Foraminotomy?
Surgery is considered when imaging and symptoms agree and proper non-surgical care has not worked.
- Arm or leg pain in one nerve's territory lasting more than 6 to 12 weeks despite physiotherapy, medication or injections
- Foraminal stenosis on MRI at the matching level and side
- Progressive weakness in the limb
- One or two affected levels without instability
It is usually not the right choice if:
- Pain mainly in the neck or back without limb symptoms
- Spinal instability or slippage, where fusion may be needed
- Central compression of the spinal cord (myelopathy), which needs a different operation
- Narrowing on a scan that causes no symptoms

- Anterior cervical discectomy and fusion: The alternative in the neck when compression comes mainly from the front or more than the foramen is involved.

The operation takes 45 to 90 minutes per level, usually under general anaesthesia. Many patients go home the same day or after one night. Limb pain is often better on waking. The wound and surrounding muscle are sore for several days, and neck surgery can cause shoulder blade spasm.

Walk from the first day. Desk work is possible after 1 to 2 weeks and driving once you can turn comfortably and are off strong painkillers. Avoid heavy lifting and twisting for 4 to 6 weeks, then start physiotherapy. Numbness and weakness recover more slowly than pain, over weeks to months, and long-standing numbness may not fully clear.

Serious problems are uncommon, but the surgery is next to a nerve root.
- Nerve root injury with new weakness or numbness
- Tear of the dura with spinal fluid leak and headache
- Wound infection or haematoma
- Incomplete relief or recurrence as arthritis progresses
- Instability if too much facet joint is removed
- C5 palsy with temporary shoulder weakness after neck surgery
