Ganglion Cysts

A ganglion is a smooth, fluid filled lump that grows out of a joint lining or tendon sheath. It contains thick clear jelly. The back of the wrist is the classic site, followed by the palm side of the wrist, the base of a finger, the end joint of a finger (mucous cyst) and the top of the foot.

Ganglions are benign and never turn into cancer. About half disappear on their own within a few years, so treatment is optional and is chosen for pain, nerve pressure or appearance.

The exact cause is unknown. The leading explanation is that a small weak point in the joint capsule or tendon sheath lets joint fluid leak out through a one way valve, where it thickens into jelly inside a sac connected to the joint by a stalk. They are most frequent in women aged 20 to 40. Cysts at the end joint of a finger are different: they occur in older people and arise from an arthritic joint with a bone spur. Repetitive use does not clearly cause ganglions, though it can make an existing one ache.

Ganglions are not emergencies. See a doctor promptly, locally, if a lump grows quickly, is hard and fixed, is hot and red, or if a finger cyst bursts and the joint becomes swollen and painful, which can indicate joint infection. Never hit a ganglion with a heavy object; it can fracture bones.

Ganglion removal is a short day case operation under regional anaesthesia, and you can fly within a few days, so it fits a brief visit or can be combined with other planned care. In about 1 in 10 people, more often at the palm side of the wrist. Recurrence after needle aspiration is much more common.

It lies next to the radial artery, so surgery needs care and aspiration is usually avoided or done under ultrasound.
