A hearing test, or audiological assessment, measures how well you hear sounds of different pitch and loudness and how clearly you understand speech. It is carried out by an audiologist in a sound-treated booth.
The test tells whether hearing loss is present, how severe it is, and whether it arises from the ear canal and middle ear or from the inner ear and nerve. It does not by itself show the cause, which may need examination or imaging.
What Hearing Test shows
Pure tone audiometry plots the quietest sounds you can hear at each pitch on a chart called an audiogram, separately for each ear, through headphones and through a bone vibrator behind the ear. The gap between the two shows conductive loss from wax, fluid, a perforation or stiff hearing bones. Speech audiometry measures how many words you repeat correctly. Tympanometry checks eardrum movement and middle ear pressure, and reflex tests check the stapes muscle. A difference between the two ears can point to the need for an MRI.
When Hearing Test is recommended
- Gradual or sudden difficulty hearing, or asking people to repeat themselves
- Tinnitus, especially in one ear
- Dizziness or balance problems
- Before and after ear surgery
- Exposure to loud noise at work, or to drugs that can harm hearing such as some chemotherapy
- Assessment for hearing aids or a cochlear implant
Limits and situations where another test is better:
- Standard audiometry needs cooperation, so babies and young children need age-adapted tests such as otoacoustic emissions, ABR or play audiometry
- It does not show tumours or structural disease; asymmetric loss needs MRI
- Ears blocked with wax give misleading results and should be cleared first
How to prepare
- Avoid loud noise such as concerts or power tools for 24 to 48 hours beforehand
- Have wax removed if your ears are blocked
- Bring your hearing aids and any previous audiograms
- Tell the audiologist if you have a cold or blocked ears on the day
What happens during the test
The full set of tests takes 30 to 45 minutes. You sit in a quiet booth wearing headphones and press a button each time you hear a tone, however faint. You then repeat words played at different volumes. For tympanometry a soft tip is held in the ear canal for a few seconds while you feel a slight pressure change. Nothing is painful.
Results and next steps
The audiologist prints the audiogram straight away and an ENT doctor explains it, usually the same day. Next steps may be wax removal, medical or surgical treatment of a middle ear problem, a hearing aid trial, or a scan if the two ears differ. Sudden loss of inner ear hearing is treated urgently with steroids.
