Joint Injection

A joint injection places medicine directly into a painful joint, or into the bursa or tendon sheath beside it. The common options are corticosteroid, hyaluronic acid and platelet rich plasma (PRP), most often for the knee, shoulder, hip, thumb base or ankle.

Injections relieve pain for weeks to months and can make rehabilitation possible. They do not rebuild cartilage or cure arthritis, and the benefit tends to shrink with repeated use and with more advanced joint damage.

What Joint Injection involves

After examination and a look at your X-rays, the skin is cleaned with antiseptic and a no touch sterile technique is used. For a knee, the needle usually enters at the outer upper edge of the kneecap with the leg relaxed and straight. If the joint is swollen, fluid is drawn off first, which itself eases pain and can be sent for analysis. The medicine is then injected; it should flow in easily. Deep joints such as the hip, and small ones such as the thumb base or facet joints, are injected under ultrasound or X-ray guidance to confirm placement. For PRP, 15 to 30 ml of your blood is taken first and spun in a centrifuge for about 10 minutes to concentrate the platelets.

Who is a good candidate for Joint Injection?

Appropriate for a flare of joint pain that limits function when tablets, exercise and weight management are not enough, or to help confirm where pain is coming from.

• Knee or hip osteoarthritis with a painful flare
• Shoulder bursitis, frozen shoulder or acromioclavicular joint pain
• Inflammatory arthritis with one or two stubborn joints
• Trigger finger and De Quervain tenosynovitis
• People who cannot have surgery or need to delay it

It is usually not the right choice if:

• Any suspicion of joint infection, or skin infection over the site
• A joint replacement planned in the next 3 months, because steroid raises infection risk
• Poorly controlled diabetes, since steroid raises blood sugar for several days
• Injection directly into the Achilles or patellar tendon, which risks rupture
• Bone on bone arthritis, where benefit is usually short

The injection takes 5 to 15 minutes in a clinic room, with no anaesthetic or a little local anaesthetic in the skin. You feel a sharp scratch and then pressure. PRP adds a blood draw and preparation time of about 30 minutes. You can walk out immediately.

One steroid injection needs a single appointment and fits into any short stay. Hyaluronic acid is given as one dose or as 3 weekly doses depending on product. PRP is often offered as 2 or 3 injections 2 to 4 weeks apart, which does not fit one short trip well, so ask whether a single injection protocol is planned or arrange later doses at home.

• Tell the doctor about medication, allergies, pregnancy or breastfeeding, and any history of cold sores, keloid scars or autoimmune disease
• Avoid alcohol, aspirin and anti-inflammatory painkillers for a few days beforehand if your own doctor agrees, to reduce bruising
• Arrive without make-up on the treatment area and avoid sunbeds and strong sun for two weeks before

Rest the joint from strenuous activity for 24 to 48 hours. A steroid flare, a day or two of increased pain, affects up to 1 in 10 people and settles with ice. PRP often causes aching for several days, and anti-inflammatory tablets are usually avoided for a week around it. Steroid works within days; PRP and hyaluronic acid take 3 to 6 weeks.

• Joint infection, roughly 1 in 10,000 or fewer, but serious
• Post injection flare of pain
• Skin thinning, dimpling or lightening at the site with steroid, mainly in superficial injections
• Temporary facial flushing and raised blood sugar after steroid
• Tendon weakening with repeated steroid near tendons
• Possible acceleration of cartilage loss with frequent repeated steroid
• No benefit
