Kidney Stone Treatment

Kidney stone treatment removes or breaks up stones in the kidney or ureter that are too large to pass, cause repeated pain or block urine flow. The main methods are shock wave lithotripsy, ureteroscopy with laser, and keyhole surgery through the back (PCNL).

Modern techniques clear most stones without open surgery. Treatment does not change your tendency to form stones: about half of people form another within 10 years unless the cause is addressed.

What Kidney Stone Treatment involves

A low-dose CT scan without contrast shows the stone's size, position and density, which decide the method. In shock wave lithotripsy you lie on a table while focused sound waves, aimed by X-ray or ultrasound, shatter the stone over 30 to 60 minutes; fragments pass in the urine over days to weeks. In flexible ureteroscopy (RIRS) a thin telescope is passed through the urethra and bladder up to the stone, a holmium or thulium laser turns it to dust, and fragments are basketed out; a temporary stent is often left. In PCNL the surgeon makes a 1 cm cut in the flank, creates a track into the kidney and removes large stones with a nephroscope.

Who is a good candidate for Kidney Stone Treatment?

Treatment is advised when a stone is unlikely to pass or is causing harm.

• Ureteric stones over about 6 mm, or smaller ones that have not passed in 4 to 6 weeks
• Kidney stones causing pain, bleeding, infection or growth
• Large or staghorn stones over 2 cm, for PCNL
• People in certain jobs, such as pilots, who cannot risk sudden colic

It is usually not the right choice if:

• Small, symptom-free stones, which can often be observed
• People with untreated urine infection, which must be cleared first
• Pregnant women, for shock wave treatment
• People on blood thinners that cannot be paused, for lithotripsy or PCNL
• Anyone with fever and a blocked kidney, who needs emergency drainage locally

Lithotripsy takes 30 to 60 minutes with painkillers or sedation and you go home the same day. Ureteroscopy takes 30 to 90 minutes under general or spinal anaesthetic, as a day case or with one night in hospital. PCNL takes 1 to 3 hours under general anaesthetic with 2 to 3 nights in hospital.

Blood in the urine and burning are normal for a few days. A stent causes urgency, frequency and flank ache when passing urine until it is removed at 1 to 2 weeks, a 5 minute procedure. Most people return to desk work within 2 to 3 days of ureteroscopy and 1 to 2 weeks after PCNL. A follow-up scan confirms clearance. Stone analysis and a 24 hour urine test guide prevention.

Risks rise with stone size and the invasiveness of the method.

• Residual fragments needing a second session
• Urinary infection or sepsis
• Stent discomfort, and harm from a forgotten stent if it is not removed on time
• Ureteric injury or later narrowing after ureteroscopy
• Bleeding needing transfusion after PCNL, in a few in every 100
• Pain as fragments pass (steinstrasse) after lithotripsy
• Injury to neighbouring organs during PCNL, which is rare
