Monsplasty

Monsplasty, also called a pubic lift, reduces and lifts the mons pubis, the fatty mound over the pubic bone. The area can become full or sagging after weight loss, pregnancy or with age, and it can look more prominent after a tummy tuck.

It flattens the bulge so clothes and swimwear sit better, and it can make hygiene easier when the tissue hangs. It will not change the labia or treat urinary problems, and further weight change can bring fullness back.

What Monsplasty involves

The surgeon marks you standing up, because the mound shifts when you lie down. If the problem is mainly fat, liposuction through two or three tiny cuts may be enough. If there is loose skin, a horizontal wedge of skin and fat is removed along the upper pubic hairline, often on the line of an old caesarean scar. The remaining tissue is lifted and anchored with deep stitches to the firm layer over the abdominal muscles so that it does not slide down again. The skin is closed in layers with dissolving stitches. It is frequently done at the same time as a tummy tuck, using the same incision.

Who is a good candidate for Monsplasty?

It suits adults at a stable weight who are bothered by a bulging or drooping pubic mound.

• You have a persistent pubic bulge despite a stable, healthy weight
• Loose pubic skin hangs after major weight loss or pregnancy
• You are having a tummy tuck and the mons would look out of proportion afterwards
• The fold causes rubbing, moisture or hygiene difficulty

It is usually not the right choice if:

• You are still losing weight or plan a pregnancy soon
• You smoke and cannot stop for at least 4 weeks either side of surgery
• You have an untreated skin infection in the groin fold
• You expect it to change sexual function or the appearance of the labia

Surgery takes 45 to 90 minutes on its own. Liposuction alone can be done under local anaesthetic with sedation; a lift is usually done under general anaesthetic. Swelling and bruising are marked for 2 weeks and can spread to the labia or scrotum, which looks alarming but settles. Desk work is possible after about a week. Wear the compression garment for 3 to 4 weeks. Avoid gym work, cycling and sex for 4 to 6 weeks. Final contour shows at 3 to 6 months as swelling goes; the scar fades over a year.

• Wound infection or slow healing in the skin fold
• Prolonged swelling of the pubic area and genitals
• Numbness or altered sensation above the pubic bone

• A widened or raised scar
• Fluid collection (seroma) needing drainage
