A pacemaker is a small battery-powered generator placed under the skin below the collarbone, connected to the heart by one to three leads. It senses each heartbeat and delivers a tiny pulse when the heart's own rhythm is too slow. An implantable cardioverter defibrillator (ICD) looks similar but can also stop dangerous fast rhythms with rapid pacing or a shock.
A pacemaker prevents blackouts and fatigue caused by slow rhythms. An ICD protects against sudden cardiac death; it does not make you feel better day to day. Neither treats blocked arteries or cures heart failure, although a special three-lead type, cardiac resynchronisation therapy, can improve pumping in selected people. Batteries last about 7 to 12 years and the generator is then changed.
What Pacemaker and Implantable Defibrillator (ICD) involves
Antibiotics are given first. Under local anaesthetic and sedation the cardiologist makes a 4 to 5 cm cut below the collarbone, usually on the non-dominant side, and forms a pocket under the skin or muscle. Leads are passed through a vein into the right ventricle and, where needed, the right atrium and a vein on the left ventricle, guided by X-ray. Each lead is tested for sensing and pacing thresholds and fixed in place. The generator is connected, placed in the pocket and the wound is closed with dissolving stitches. Devices are programmed wirelessly and checked again before discharge with a chest X-ray.
Who is a good candidate for Pacemaker and Implantable Defibrillator (ICD)?
Indications follow international guidelines and rest on ECG, Holter monitoring, echocardiography and your history.
- Complete or high-grade heart block
- Sick sinus syndrome with symptoms such as fainting or exhaustion
