A penile implant (penile prosthesis) is a device placed inside the erection chambers of the penis so that a man with erectile dysfunction can have a rigid erection on demand. Types are the inflatable three-piece pump system and the simpler bendable (malleable) rods.
Satisfaction is high, at around 85 to 90 in 100 for men and partners. It is irreversible: natural erections do not return afterwards. It does not add length, and sensation, orgasm and ejaculation are unchanged.
What Penile Implant involves
Through a small cut at the junction of penis and scrotum, or just above the penis, the surgeon opens each of the two corpora cavernosa, dilates and measures them, and inserts a cylinder of matching length. For an inflatable implant, a pump is placed in the scrotum and a saline reservoir behind the abdominal wall, all joined by tubing under the skin. Squeezing the pump moves fluid into the cylinders to give an erection; a release valve lets it down. A malleable implant has two firm rods that are simply bent up or down. Strict infection precautions are used, including antibiotic-coated devices. The device is left partly inflated at first and activated at about 4 to 6 weeks.
Who is a good candidate for Penile Implant?
It is for men with lasting erectile dysfunction when tablets, injections or vacuum devices have failed or are not acceptable.
- Erectile dysfunction from diabetes, vascular disease or after prostate cancer treatment
- Men who do not respond to or cannot take PDE5 inhibitors
- Peyronie's disease with erectile dysfunction, where the implant also straightens the penis
- After priapism or pelvic injury with permanent damage
It is usually not the right choice if:
- Men whose erectile dysfunction is psychological or potentially reversible
- Poorly controlled diabetes (high HbA1c), until it is improved
- Active infection anywhere, including urinary or skin infection

At 4 to 6 weeks you are taught to inflate and deflate the device, and sex can resume once that is comfortable. Remote teaching by video is possible, but an in-person check is better.

- Pump or reservoir migration
- Persistent pain
- Cool or soft glans during erection
