Scar revision surgery improves a scar that is wide, raised, sunken, tight or badly placed. The old scar is cut out or rearranged and the wound is closed again under better conditions.
It can make a scar thinner, flatter, less tight and better hidden in natural skin lines. It cannot erase a scar: you exchange one scar for a better one. Keloids in particular tend to return unless surgery is combined with other treatment.
What Scar Revision Surgery involves
The scar is assessed for type, direction relative to skin tension lines, and tightness. In simple fusiform excision the scar is removed as an ellipse and the edges are closed in layers, with deep stitches taking the tension so that the surface heals as a fine line. A Z-plasty or W-plasty breaks a straight scar into small angled segments; this lengthens a tight scar and turns it towards natural creases. Very wide scars are removed in stages months apart, or after tissue expansion. Sunken scars may be released underneath and filled with fat. Afterwards, silicone sheets, taping, steroid injections or laser help the new scar mature well.
Who is a good candidate for Scar Revision Surgery?
Surgery is for mature scars, generally at least 12 months old.
- Your scar is wide, stepped or runs across natural skin lines
- A tight scar restricts movement of a joint, eyelid or lip
- The scar is sunken or tethered to deeper tissue
- Non-surgical measures such as silicone, steroid injection and laser have not helped enough
It is usually not the right choice if:
- The scar is under 6 to 12 months old and still red; many improve on their own
- You have a keloid and cannot attend for steroid injections or radiotherapy afterwards
- You smoke or have poorly controlled diabetes

Avoid stretching the area for 6 weeks. The new scar looks red and firm at first, peaks at about 2 months and then softens. Judge it at 12 months, and protect it from sun throughout.

The main risk is that your skin heals in the same way as it did before.
- The new scar stretching or thickening again
- Keloid recurrence, sometimes larger than the original

How old must the scar be?

Can keloids be cut out?

Is laser better than surgery?
They do different jobs. Laser improves colour and texture; surgery changes width, direction and tightness.
Can I travel home soon after?
