Sclerotherapy

Sclerotherapy treats spider veins and small varicose veins by injecting a solution that irritates the vein lining, so the vein closes, is absorbed and fades from view.

It has been the standard treatment for leg thread veins for decades. Larger varicose veins usually need the underlying faulty vein treated first, which is why a duplex ultrasound scan comes before cosmetic injections.

What Sclerotherapy involves

With you lying down, the doctor injects a sclerosant such as polidocanol or sodium tetradecyl sulphate through a very fine needle directly into the veins. For larger veins the solution is mixed with air into a foam and guided by ultrasound. Compression stockings are put on straight afterwards.

Who is a good candidate for Sclerotherapy?

An ultrasound scan of the leg veins decides whether injections alone are enough.

• Spider veins and small blue reticular veins on the legs
• Small varicose veins without reflux in the main trunk veins
• Veins remaining after laser or surgery for varicose veins
• Able to walk daily and wear compression stockings afterwards

It is usually not the right choice if:

• Pregnancy or breastfeeding
• A history of deep vein thrombosis or a clotting disorder, without specialist assessment
• Unable to walk, or poor arterial circulation in the legs
• Untreated reflux in the main veins, because new veins will keep appearing
• A long flight planned immediately afterwards

A session takes 15 to 45 minutes. The injections sting or cramp briefly. You walk for 15 to 30 minutes straight after, which is part of the treatment.

One to three sessions are needed, four to six weeks apart. Long-haul travel soon after sclerotherapy raises the question of blood clots; most doctors ask you to avoid long flights for a period afterwards, often one to two weeks after treatment of larger veins. Agree the timing with your doctor before you book flights.

• Tell the doctor about medication, allergies, pregnancy or breastfeeding, and any history of cold sores, keloid scars or autoimmune disease
• Avoid alcohol, aspirin and anti-inflammatory painkillers for a few days beforehand if your own doctor agrees, to reduce bruising
• Arrive without make-up on the treatment area and avoid sunbeds and strong sun for two weeks before

Wear compression stockings for one to two weeks as advised, walk daily, and avoid hot baths, saunas and heavy leg exercise for a week. Veins often look worse before they improve. Spider veins fade in three to six weeks and larger veins in three to four months.

• Back to everyday activity: None; stockings for 1 to 2 weeks
• When results show: 3 to 6 weeks for spider veins, 3 to 4 months for larger
• How long they last: Treated veins stay closed; new ones can form

Common effects are cosmetic and temporary. Serious ones are rare:

• Bruising, tender lumps of trapped blood and itching
• Brown staining along the vein, which can take months to fade
• Matting, a blush of new fine vessels near the treated area
• Skin ulceration if solution leaks outside the vein
• Deep vein thrombosis, allergic reaction or, with foam, brief visual disturbance or migraine
