Sinus Lift (Maxillary Sinus Augmentation)

A sinus lift adds bone to the back of the upper jaw so that dental implants can be placed there. After upper molars are lost, the jawbone shrinks and the air-filled maxillary sinus expands downwards, leaving only a few millimetres of bone. The surgeon gently raises the sinus lining and packs bone graft into the space created beneath it.

It makes secure implants possible where they otherwise would not be. It adds healing time, often 4 to 9 months before teeth can be fitted, and is not needed if short, angled or zygomatic implants can avoid the sinus.

What Sinus Lift (Maxillary Sinus Augmentation) involves

A cone beam CT scan measures the remaining bone height and checks the sinus for disease. Under local anaesthesia, the gum is lifted from the side of the upper jaw. In the lateral window technique, a small oval window is outlined in the thin bone with a piezoelectric or rotary instrument, and the Schneiderian membrane lining the sinus is carefully peeled upwards like a tent. Graft material is packed beneath it, the window is covered with a collagen membrane and the gum is stitched. In the crestal technique, used when at least 5 to 6 millimetres of bone remain, the sinus floor is tapped upwards through the implant socket and the implant is placed in the same visit.

Who is a good candidate for Sinus Lift (Maxillary Sinus Augmentation)?

You may need a sinus lift if a scan shows too little bone height under the sinus for a standard implant.

• People missing upper premolars or molars with less than about 8 millimetres of bone below the sinus
• Patients planning full-arch upper implants who prefer conventional implants to zygomatic ones
• Healthy sinuses with no ongoing infection
• Non-smokers or those who can stop around the surgery

It is usually not the right choice if:

• Active or chronic sinusitis, polyps or cysts blocking sinus drainage, which need ENT treatment first
• Heavy smokers, in whom graft and implant failure are clearly more frequent
• Uncontrolled diabetes, or recent high-dose radiotherapy to the upper jaw
• People on intravenous bisphosphonates or similar bone drugs, because of jaw necrosis risk

For 2 to 3 weeks do not blow your nose, sneeze with your mouth open, and avoid straws, diving and playing wind instruments. Minor nosebleeds in the first days are normal. Stitches come out at 7 to 14 days. The graft matures over 4 to 9 months before implants are placed or loaded.

• An opening between the mouth and sinus (oroantral fistula), which is uncommon
