Wisdom teeth are the third molars, which normally appear between the ages of 17 and 25. Many jaws lack room for them, so they stay partly or fully buried, tilted against the tooth in front. Removal is the surgical extraction of one or more of these teeth.
Taking out a troublesome wisdom tooth ends repeated gum infections and protects the neighbouring molar from decay. Healthy, symptom-free wisdom teeth generally do not need removing, and lower wisdom teeth lie near nerves that supply feeling to the lip, chin and tongue, so the decision should weigh benefit against that risk.
What Wisdom Teeth Removal involves
A panoramic X-ray shows the tooth's angle, depth and root shape. If the roots overlap the nerve canal, a cone beam CT scan clarifies the relationship. After local anaesthetic has taken effect, the surgeon lifts a small flap of gum, removes a collar of bone with a cooled drill and usually divides the tooth into two or three pieces so it can be delivered through a small opening. The socket is cleaned and rinsed and the gum closed with dissolving stitches. Upper wisdom teeth that have erupted often come out with forceps in a few minutes. If the scan shows roots wrapped around the nerve, a coronectomy may be offered, removing the crown and deliberately leaving the roots.
Who is a good candidate for Wisdom Teeth Removal?
Removal is advised when a wisdom tooth is causing, or is very likely to cause, disease.
- Repeated pericoronitis, infection of the gum flap over a partly erupted tooth
- Decay in the wisdom tooth or in the back of the second molar
- A cyst or bone loss associated with the tooth
- An upper wisdom tooth biting into the cheek or lower gum
- Removal requested before jaw surgery, radiotherapy or certain medicines
It is usually not the right choice if:
- Fully buried, symptom-free teeth with no disease on X-ray, which can simply be monitored
- Removal to prevent front tooth crowding, which evidence does not support
- Active severe infection with restricted opening, treated first with drainage and antibiotics
- Uncontrolled bleeding disorders or unstable medical conditions until managed
- Older adults with deeply buried teeth close to the nerve and no symptoms
الخيارات التقنية
- Simple forceps extraction: For erupted teeth with favourable roots.
- Surgical extraction with sectioning: The standard approach for impacted lower teeth.
- Coronectomy: Leaves the roots in place when they are intimately related to the inferior alveolar nerve.
- Piezoelectric bone surgery: Ultrasonic cutting that is gentler near nerves, though slower.
- All four under sedation or general anaesthesia: A single session for people needing several difficult teeth removed.
ماذا يحدث خلال علاجك
A single tooth takes 5 to 40 minutes depending on difficulty; all four take about an hour. Most are done under local anaesthetic, with intravenous sedation if you prefer. You feel firm pressure and hear the drill and cracking sounds, but no sharp pain. Numbness lasts 3 to 4 hours.
الإعداد لرحلتك
Plan for 4 to 7 days in Istanbul. Your surgeon reviews photographs and your medical history before you book, and you meet in person the day before surgery for examination, marking and consent with a professional interpreter.
- Stop smoking and nicotine at least four weeks before and after surgery; it is the single biggest avoidable cause of wound problems
- Tell the team about every medicine and supplement you take; blood thinners, some herbal products and hormone treatment may need to be paused on your doctor's advice
- Arrange for someone to travel with you or to be reachable, and keep the first days at home free of work and lifting
- Book a changeable return flight; your surgeon confirms when you are fit to fly
التعافي والنتائج
Swelling and jaw stiffness peak at 48 to 72 hours and largely settle within a week. Take regular ibuprofen and paracetamol, use ice packs on the first day and eat soft, cool food. Do not rinse, spit forcefully or smoke for 24 hours; then use warm salt water after meals. Most people take 2 to 4 days off work. Stitches dissolve in 1 to 2 weeks and the socket closes over about a month.
- Back to everyday activity: 2 to 5 days
- When results show: Symptoms resolve as the socket heals over 2 to 4 weeks
- How long they last: Permanent
سياسة السلامة والمخاطر والتنقيح
Complications are mostly temporary, but nerve injury needs to be discussed properly before lower tooth surgery.
- Dry socket, a throbbing pain starting on day 2 to 4, more common in smokers and with the contraceptive pill
- Infection and delayed healing
- Numbness or tingling of the lip, chin or tongue, usually temporary and permanent in well under 1 in 100 cases
- Limited mouth opening for one to two weeks
- Bleeding
- Damage to the neighbouring tooth or its filling
- An opening into the sinus after upper tooth removal
- Jaw fracture, which is very rare

- Number of teeth and depth of impaction

الأسئلة المتكررة
Do all wisdom teeth have to come out?
No. Those that are healthy, fully erupted and cleanable, or deeply buried without disease, can be left and checked periodically.
Is it better to remove all four at once?
If all four need removing, one session means one recovery. Doing two sides separately lets you chew on the other side.
When can I fly?
Usually after 2 to 3 days. Staying about 5 to 7 days lets the surgeon deal with a dry socket, which typically appears in that window.
What is a dry socket?
The blood clot is lost and bone is exposed. It is painful but not dangerous and is treated with a medicated dressing.
