HoLEP (holmium laser enucleation of the prostate) treats benign prostate enlargement. Working through the urethra, the surgeon uses a laser to shell out the overgrown inner part of the gland in one piece, leaving the outer capsule.
It gives strong, lasting relief of a weak stream and incomplete emptying, and suits prostates of any size, including very large ones. Its main trade-off is that nearly all men have dry (retrograde) ejaculation afterwards.
What Prostate Laser Treatment (HoLEP) involves
Under anaesthetic a resectoscope is passed along the urethra. The surgeon uses the holmium laser fibre to find the plane between the enlarged adenoma and the prostate capsule, then peels each lobe away from the capsule, sealing blood vessels as they are met. The freed lobes are pushed into the bladder. A second instrument called a morcellator then cuts the tissue into fragments and suctions them out, and these go to pathology. A catheter with bladder irrigation is left in, usually for 24 to 48 hours. There are no skin incisions.
Who is a good candidate for Prostate Laser Treatment (HoLEP)?
It is for men with bothersome urinary symptoms from benign enlargement when medicines are not enough.
- Moderate to severe symptoms despite alpha blockers or 5-alpha reductase inhibitors
- Urinary retention needing a catheter
- Large prostates over 80 ml that would otherwise need open surgery
- Men on blood thinners, since laser bleeding is low
- Bladder stones, recurrent infections or kidney strain caused by obstruction
It is usually not the right choice if:
- Men for whom preserving ejaculation is a priority, who may prefer other techniques
- Untreated urine infection
- Symptoms caused mainly by an overactive or failing bladder rather than obstruction

Desk work is possible after about a week.
